C. Such order shall be in substantially the following <br /> form: <br /> TO: <br /> ORDER TO CORRECT UNSAFE CONDITION <br /> An unsafe condition (as defined in Section <br /> of Ordinance ) affecting <br /> the City of Everett Surface Water System <br /> exists on the premises at (address) <br /> .
