AMENDMENT NO. 1 TO PROFESSIONAL SERVICES AGREEMENT BETWEEN <br /> THE CITY OF EVERETT <br /> AND <br /> Dykeman <br /> The City and Dykeman agree to amend and modify their professional services agreement dated <br /> August 9, 2010 (the"Agreement") as follows: <br /> WHEREAS, the city desires to engage the contractor to prepare construction documents and <br /> provide limited contract administration services for a new Teen Area and Story Time Room at <br /> the Main Library and <br /> WHEREAS, additional work has been added to the scope of work and the need to hire an <br /> Electrical and Mechanical Engineer have become apparent; and <br /> WHEREAS, A need exists to write bid specifications including mechanical and electrical <br /> sections, and architectural specification sections; and <br /> WHEREAS, the total amount of compensation stated in the original agreement is insufficient for <br /> the revised scope of the project; and <br /> WHEREAS, the City and Dykeman agree to amend their Agreement to: (a) extend the time for <br /> completion from September 1, 2011 to October 31, 2011 and(b) increase the maximum <br /> compensation by an additional $11,760.00 bringing the total compensation to $27,340.00; <br /> NOW THEREFORE, the City of Everett and Dykeman agree to amend and modify the <br /> previously amended Agreement as follows: <br /> 1. The following work is added to the Scope of Work described in Exhibit A of the <br /> Agreement: Write specifications including mechanical and electrical sections, in addition to <br /> architectural specification sections, detail the through-wall penetration at the Heat Pump unit, <br /> complete additional detail on drawings for competitive bid, select paint colors in children's area <br /> and review with Library staff, and denote and detail shelving tops and end panels <br /> 2. Paragraph 4(D)is modified to read as follows: <br /> Total compensation, including all services and expenses, shall not exceed a <br /> maximum of Twenty Seven Thousand Three Hundred Forty Dollars <br /> ($27,340.00). <br /> 3. Dykeman will provide the City Of Everett with the following as they relate to the new <br /> Teen Area and Story Time Room at the Main Library: Specification Section for all work to be <br /> completed, hire Mechanical and Electrical engineers to write/review Specification Sections, <br /> provide drawings for the shelf tops and end panels for cut down book stacks(which will include <br /> quantities of tops, end panels and stacks to be cut, plus dimensions), select all paint colors and <br /> C:\Documents and Settings lspattiso\Desktop\Library Children\Amendment Form doe(YJ 7/15/1999) <br /> 8 <br />
