• <br /> 12 <br /> Construction expenses incurred after August 3, 2011, are eligible for reimbursement. Requests for <br /> reimbursements for costs related to construction activities will not be accepted until the Contractor has . <br /> met the following conditions: <br /> A. Issued a Notice to Proceed which follows the formal award of a construction contract; <br /> B. Completed the State Environmental Review Process; <br /> C. Complied with all provisions of.Section 106 of the.National Historic Preservation Act of 1966; <br /> D. Complied with Section 1.19.Prevailing Wage; <br /> E. Obtained approval from the Department of,Health of the project report and related construction <br /> documentsfor all applicable"activities described in Attachment I: Scope of Work; and <br /> F. Complied with any other loan conditions required by Department of Health or the Board. <br /> The Contractor shall submit all Invoice Vouchers and all required documentation to: <br /> Contracts Administration Unit <br /> • Department of Commerce <br /> PO Box 42525 <br /> Olympia,WA 98504-2525 - <br /> The Board will pay the Contractor upon acceptance of the work performed and receipt of properly <br /> completed invoices. Invoices may be submitted to the Board not more often than monthly. <br /> Payment shall be considered timely if made by the Board within thirty(30)calendar days after receipt of <br /> properly completed invoices. Payment shall be sent to the address designated by the Contractor. <br /> The Board may, at its sole discretion, terminate the contract or withhold payments claimed by the <br /> Contractor for services rendered if the Contractor fails to satisfactorily comply with any term or condition <br /> of this"contract. <br /> • <br /> No payments in advance or in anticipation of services or supplies to be provided under this contract shall <br /> be made by the Board. <br /> In the event that the Contractor receives reimbursement for costs that are later determined by the Board <br /> to be ineligible, these funds shall be repaid to the Drinking Water Assistance Account by payment to the <br /> Department of Commerce, or its successor,together with the submission of the Project Completion <br /> Amendment. <br /> • <br /> At the time of projectcompletion, the Contractor shall submit to the Board a Certified Project Completion <br /> Report certifying the total actual project casts, and a final voucher for the remaining eligible funds. The <br /> Certified Project Completion.Report shall include a copy of the Construction Completion Report as <br /> submitted to Department of Health. <br /> 1.10. TIME OF PERFORMANCE <br /> The Contractor shall begin the activities identified within Attachment I: Scope of Work no later than thirty(30) <br /> days after Contract execution.
