B. The License Director may recommend to the Secretary that a <br /> licensee's certificate/license be revoked or suspended if any of the <br /> following have occurred: <br /> 1 . If the license holder is convicted of, or forfeits bail <br /> for, any of the following driving violations whether or not <br /> in connection with driving an ambulance: <br /> a. Reckless driving. <br /> b. Driving under the influence of intoxicating liquor or <br /> drugs. <br /> 2. If the license holder is convicted of any felony or any <br /> offense involving moral turpitude. <br /> 3. If the license was obtained by fraud or if the application <br /> therefor contains any false statements, or if a fact <br /> concerning the applicant was not disclosed at the time of <br /> his making application when such fact would have <br /> constituted just cause for refusing to issue such license. <br /> 4. If the licensee fails to comply with the provisions of this <br /> ordinance. <br /> C. The initial , semi-annual , or other ambulance and/or emergency <br /> first aid vehicle, equipment and premise inspection reports of the Medical <br /> Director herein provided for shall be prima facie evidence of compliance or <br /> non-compliance with, or violation of, the provisions, standards and <br /> requirements provided herein, and ambulances and/or emergency first aid <br /> vehicles. <br /> D. Upon suspension, revocation, or termination of an ambulance <br /> and/or emergency first aid vehicle license hereunder operations as such <br /> shall cease with such vehicle and no person shall permit continued operation <br /> with such ambulance and/or emergency first aid vehicle as such. <br /> E. Upon suspension, revocation or termination of any other <br /> license/certificate issued by the Secretary, such person shall cease to <br /> drive or attend an ambulance and/or emergency first aid vehicle and no <br /> person shall employ or permit such individual to drive or attend an <br /> ambulance and/or emergency first aid vehicle. <br /> SECTION 9: Reports. <br /> A. Each licensee of an ambulance and/or emergency first aid vehicle <br /> hereunder shall maintain accurate records, upon such forms as may be <br /> 6 <br />
