I <br /> City of Everett Human Needs Grant <br /> January 1 through December 31, 2017 <br /> Arc of Snohomish County <br /> Independent Living/Counseling& Guidance; Emergency Housing-Shelter& Food; and Health Services <br /> EXHIBIT A: Program Summary <br /> Counseling&Guidance: <br /> The intent of this program is to provide support and advocacy to individuals with intellectual and developmental <br /> disabilities(I/DD) and their families. With our support, programs and workshops are designed to.give individuals <br /> the tools they need to connect to resources in the community and gives them power and choice allowing them to <br /> live as independently as possible. The Arc strives to keep individuals with I/DD safe and their families educated — <br /> with up to date information and resources ensuring a successful future. By providing this information and the <br /> support to navigate very confusing systems, our most vulnerable citizens and their families are more likely to <br /> plan for the future giving their loved one with a disability every chance for success. <br /> Emergency Housing-Shelter/Food: <br /> The intent of this program is to provide emergency assistance for individuals with intellectual and developmental <br /> disabilities(I/DD)to maintain their living situation by supporting them with utilities, rent and food when in a <br /> crisis situation. Direct case management will be provided through other Arc agency funding to connect <br /> individuals to the proper community systems and provide the advocacy necessary for them to retain their <br /> housing situation and be safe and independent in the community. <br /> Health Services: <br /> With continued changes to our health care systems, and the threat of more change to Medicaid and Medicare <br /> programs that many individuals with intellectual and developmental disabilities rely on,the Arc helps support <br /> individuals with I/DD and their families finding appropriate medical and dental care providers that will accept <br /> Medicaid and Medicare. It is often challenging to connect to specialty care clinics that have an understanding of <br /> a person's diagnosis. We also advocate on behalf of individuals with I/DD to retain the insurance coverage they <br /> have. <br />
