Project File M&N#:8057-03 <br /> 3 March 2015 Site Visit Memorandum <br /> As noted in the Moffatt& Nichol field report dated 2 March 2015,tieback 16 appeared to be damaged <br /> in two locations. Moffatt & Nichol inspected the condition of the grout in these two locations after the <br /> sheathing had been removed. The damage is shown in Figures 5 and 6. Figure 5 shows the damaged <br /> area where the ecology block came into contact with the tierod. Only the sheathing was damaged in this <br /> location,the grout was undamaged.The contractor will repair this area with HDPE tape per the tieback <br /> manufacturers recommendations. <br /> Figure 6 shows the damaged area near the tieback. At this location the tieback looked deformed <br /> possibly indicating that the grout had been crushed. After the sheathing had been removed it was found <br /> that the grout was undamaged and uncracked. It appears this portion of the tieback had slightly <br /> deformed sheathing before the grout was poured.This portion of the tieback will be repaired using <br /> HDPE tape. <br /> "„ 6 <br />
