City of Everett Human Needs Grant January 1,2017 through December 31,2017 Exhibit B Terms,Conditions and Projected Budget Organization: Bridgeways Program: Emerald Apartments&Bridgeways Place Apartment Permanent Housing Will spend the funds received for the specific purposes of: Funding of a Housing Coordinator for increasing self—sufficiency of the tenants and working with prospective tenants. Our housing coordinator makes regular contact with tenants to encourage collaboration among tenants to promote using life skills/tools by facilitating a bi-monthly group meeting. Our housing coordinator uses motivational interviewing to encourage change among tenants in the direction of health and independence. The housing coordinator and landlord are in contact with tenants to help rent is paid when due,and if the need arises to work with them on payment plans. Our housing coordinator is available to tenants as a resource to access information and services relevant to any needs or difficulties they are experiencing. For the purposes of this grant, a unit of service is defined as: A Bednight Bridgeways anticipates this grant will fund a total of 4800 units of service for(unduplicated count) 16 Everett residents each year (define period—month,quarter, etc.) Bridgeways anticipates its budget for the year 1/1/2017 through 12/31/2017, by quarters,will be: First Qtr.
