City of Everett Human Needs Grant <br /> January 1 through December 31, 2017 <br /> Lutheran Community Services Northwest <br /> Healthy Families Project: Parent and Youth Empowerment <br /> EXHIBIT A: Program Summary <br /> Journeys Out of Poverty(JOP) is a systematic, long-term model of support that breaks cycles of generational <br /> poverty and moves families from crisis to self-sufficiency. Working one-on-one with people who are low-income <br /> and homeless,our Resource and Support Specialists(RSS's)help participants connect to resources and supports <br /> including: permanent housing, full-time employment, education and training opportunities, mental health <br /> counseling, drug/alcohol rehabilitation, health care,domestic violence intervention,parenting education and <br /> financial literacy coaching that help them to overcome barriers and achieve lasting health and stability. <br /> Working one-on-one with our Resource Support Specialists(RSS's), participants transition from crisis to health <br /> and stability as outlined below: <br /> 1.)Participants meet with an RSS either by appointment or on a drop-in basis to describe their situation and go <br /> through our registration process. <br /> 2.)Connection to basic needs and support services such as housing, counseling and rehabilitation help to stabilize <br /> participants. <br /> 3.)Regular one-on-one follow up, advocacy and coaching teaches participants new skills,behaviors and avenues <br /> for approaching problems to build their"middle class toolbox."These include life skills,hard and soft job skills <br /> (interviewing, computer skills,and customer service),managing finances and parenting skills, all of which can be <br /> transferred to children and the entire family,thus breaking cycles of generational poverty. <br /> 4.)Connection to family members, support groups,counselors, faith communities and other networks increases a <br /> sense of belonging,which numerous studies have shown increases family resilience. <br /> Specific programs included under this grant include: <br /> • Journeys Out of Poverty: One-on-one crisis and basic needs assistance, information and referral, <br /> connection to resources, coaching, systems navigation and ongoing support <br /> • National Night Out Against Crime Community Event <br />
