EVERETT PARKS AND RECREATION of City of Everett 802 Mukilteo Blvd EVERETT WA UNITED STATES 98203 (hereinafter"the holder"), is hereby authorized to use and occupy National Forest System lands,subject to the terms and conditions of this permit,to provide outfitting and guiding services within the Mt. Baker, Darrington, Skykomish, Snoqualmie, Hood Canal, Cowlitz Valley, and Mt.Adams Ranger Districts and Mt. St. Helens NVM of the MT BAKER- SNOQUALMIE NATIONAL FOREST, GIFFORD PINCHOT NATIONAL FOREST, OLYMPIC NATIONAL FOREST, described as and as shown on the map of the authorized area, attached as Appendix A.The above-described area shall be referred to as the"permit area." The purpose of this permit is to authorize the following outfitting and guiding activities: Day hiking,backpacking,canoeing,cross country skiing and snowshoeing on the Mt Baker Snoqualmie and Olympic National Forests. and assign use as follows: 480 priority use service days for Mt. Baker Snoqualmie National Forest. 50 priority use service days for Olympic National Forest This use will be exercised as described in the five-year operating plan. The following appendices are attached to and made a part of this permit: APPENDIX A-Map of Authorized Area APPENDIX B-Five-year Operating Plan, dated December 2016 and approved annually APPENDIX C-Trip Itinerary APPENDIX D-Estimated Fee Determination Sheet, approved annually APPENDIX E-Actual Use Report Format APPENDIX F-Authorized Officer's Outfitter and Guide Performance Evaluation Criteria APPENDIX G-Other Tips for camping in bear country I.AUTHORITY AND GENERAL TERMS OF THE PERMIT A.AUTHORITY.This permit is issued pursuant to Federal Lands Recreation Enhancement Act, 16 U.S.C.6802(h), and 36 CFR Part 251, Subpart B,as amended, and is subject to their provisions. B.AUTHORIZED OFFICER.The authorized officer is the Forest Supervisor or a subordinate officer with delegated authority. C.TERM.This permit shall expire at midnight on 12/31/2026. Expiration of this permit shall not require notice, a decision document, or any environmental analysis or other documentation. D.RENEWAL.This permit may be renewed upon expiration, provided the use is consistent with the applicable forest land and resource management plan, applicable laws and regulations,and the terms of this permit, and the holder has performed satisfactorily under this permit, as demonstrated by acceptable annual performance reviews. If the holder wants
