DESCRIPTION/JUSTIFICATION OF AMENDMENT, MODIFICATION,OR CHANGE ORDER: <br /> The Department of Homeland Security(DHS),Federal Emergency Management Agency(FEMA)is providing funds to the state and the specific urban area of <br /> the City of Seattle,City of Bellevue,King County,Snohomish County,and Pierce County to enhance their capacity to prevent terrorist attacks involving CBRNE <br /> devices and to meet the unique needs of high urban threat areas through the FFY 10 Urban Area Security Initiative Program(UASI). <br /> The UASI Core Group and Snohomish County allocated Urban Area Security Initiative FFY 2010(UASI-10) grant <br /> funding for the purchase of a CBRNE Heavy Rescue Fire Truck. Snohomish County has identified additional <br /> UASI-10 funding for the purchase and installation of additional specialized equipment to be attached to this fire <br /> truck. <br /> Amendment A also includes a two month extension to allow more time to meet the added contract deliverables. <br /> 11. AMENDMENT TERMS AND CONDITIONS: <br /> 1. Change the contract expiration date of April 30,2013 to June 30,2013. <br /> 2. Increase in the overall contract award by$170,000 from$600,000 to$770,000. <br /> 3. Replace Exhibit C—Budget Sheet,with Exhibit C—Budget Sheet Amendment A to add$170,000 to the Equipment budget. <br /> No other changes are required. All other terms and conditions of the original contract and any previous amendments thereto remain in full force and <br /> effect. <br /> Snohomish County and Contractor acknowledge and accept the terms of this contract amendment and attachments hereto and have executed this <br /> amendment as of the date and year written below. The rights and obligations of both parties to this contract are governed by this Contract Amendment <br /> Face Sheet and other documents incorporated herein by reference or attached and identified in the original contract document. A copy of this contract <br /> amendment shall be attached to and incorporated into the original agreement between the Department and the Contractor.
