I <br /> iDIVISION P—PROPOSAL <br /> I CITY OF EVERETT,WASHINGTON <br /> HAYES STREET REGULATOR AND CSO CONTROLS (SRO1, SRO2,AND SRO3) <br /> IWORK ORDER#UP 3398-31 <br /> aTo the City Council <br /> Everett, Washington <br /> I <br /> that he has carefullyexamined the Plans and Specifications, <br /> The undersigned Bidder declares p <br /> Notice to Contractors, Standard Specifications, Special Provisions, Appendix, Proposal, and <br /> Contract for the construction of approximately 550 LF of new combined sewer pipe (30-inch to <br /> 72-inch dia.); removal of approximately 325 LF of existing combined sewer pipe (30-inch to 48- <br /> inch dia.); construction of five (5) separate underground flow regulator, screening, or monitoring <br /> IIIconcrete vaults; structural modifications within a large existing underground concrete siphon- <br /> pipe headworks facility, including demolition of the above-ground entry structure; manhole <br /> furnish and install mechanical screen equipment system at lift station #9; installation of <br /> temporary flow diversion and bypass systems; approximately 180 LF of existing 8-inch gravity <br /> sanitary sewer relocation and service re-connects; electrical power and control ductbanks; <br /> construction of approximately 600 LF of new 24-foot wide paved access drive; existing street <br /> and site asphalt pavement restoration; permanent fence installation, landscaping and performing <br /> all such Work as may be necessary, in accordance with the Specifications, as shown on the <br /> Plans. The undersigned declares that he has made such investigations as are necessary to <br /> Idetermine the conditions to be encountered, and that, if this Proposal is accepted, he will enter <br /> into a contract with the City of Everett, Washington, in the form of Contract hereto annexed, he <br /> I will, to the extent required, provide the necessary equipment, tools, apparatus, and other means <br /> of construction, and he will furnish all labor and materials necessary to complete the Work in the <br /> manner herein specified and according to the requirements of the Engineer. <br /> I The undersigned Bidder certifies that this Proposal is in all respects fair and is made without <br /> collusion on the part of any person, firm or corporation mentioned below, and no officer or <br /> employee of the City of Everett is personally or financially interested, directly or indirectly, in <br /> the Proposal or in any purchase of or sale of any materials or supplies for the Work to which it <br /> relates, or any portion of the profits thereof <br /> i The undersigned Bidder agrees that he will complete the Work in all respects as required by <br /> Division C, Article 2. Time for Completion and that he will pay liquidated damages to the City <br /> in the amount specified in the Contract. <br /> IAccompanying this Proposal is a bid bond or certified check in the amount of five percent (5%) <br /> I of the Proposal according to the conditions of the `Notice to Contractors" and "Division 1 — <br /> General Requirements" hereby attached.
