I iDIVISION P—PROPOSAL I CITY OF EVERETT,WASHINGTON HAYES STREET REGULATOR AND CSO CONTROLS (SRO1, SRO2,AND SRO3) IWORK ORDER#UP 3398-31 aTo the City Council Everett, Washington I that he has carefullyexamined the Plans and Specifications, The undersigned Bidder declares p Notice to Contractors, Standard Specifications, Special Provisions, Appendix, Proposal, and Contract for the construction of approximately 550 LF of new combined sewer pipe (30-inch to 72-inch dia.); removal of approximately 325 LF of existing combined sewer pipe (30-inch to 48- inch dia.); construction of five (5) separate underground flow regulator, screening, or monitoring IIIconcrete vaults; structural modifications within a large existing underground concrete siphon- pipe headworks facility, including demolition of the above-ground entry structure; manhole furnish and install mechanical screen equipment system at lift station #9; installation of temporary flow diversion and bypass systems; approximately 180 LF of existing 8-inch gravity sanitary sewer relocation and service re-connects; electrical power and control ductbanks; construction of approximately 600 LF of new 24-foot wide paved access drive; existing street and site asphalt pavement restoration; permanent fence installation, landscaping and performing all such Work as may be necessary, in accordance with the Specifications, as shown on the Plans. The undersigned declares that he has made such investigations as are necessary to Idetermine the conditions to be encountered, and that, if this Proposal is accepted, he will enter into a contract with the City of Everett, Washington, in the form of Contract hereto annexed, he I will, to the extent required, provide the necessary equipment, tools, apparatus, and other means of construction, and he will furnish all labor and materials necessary to complete the Work in the manner herein specified and according to the requirements of the Engineer. I The undersigned Bidder certifies that this Proposal is in all respects fair and is made without collusion on the part of any person, firm or corporation mentioned below, and no officer or employee of the City of Everett is personally or financially interested, directly or indirectly, in the Proposal or in any purchase of or sale of any materials or supplies for the Work to which it relates, or any portion of the profits thereof i The undersigned Bidder agrees that he will complete the Work in all respects as required by Division C, Article 2. Time for Completion and that he will pay liquidated damages to the City in the amount specified in the Contract. IAccompanying this Proposal is a bid bond or certified check in the amount of five percent (5%) I of the Proposal according to the conditions of the `Notice to Contractors" and "Division 1 — General Requirements" hereby attached.
