I <br /> submitted along with a Statement of Compliance affidavit photo copied from the back of <br /> form WH-347, or equivalent. <br /> I The first payroll submitted for the Work for both the Contractor and each Subcontractor <br /> shall be labeled"Initial." The last payroll submitted for the Work for both the Contractor <br /> and each Subcontractor shall be labeled "Final." Payrolls shall be sequentially numbered <br /> for all periods in which Work has been done. A certificate of completion for the Work, <br /> Isigned by the City,will constitute acceptance of the Work. The issuance of this certificate <br /> of completion will not constitute acceptance of unauthorized or defective Work or <br /> material is performed. <br /> 1 1-07.11 Requirements for Nondiscrimination <br /> Supplement 1-07.11 as follows: <br /> I The Contractor will be required to assure that equal employment opportunities will be in effect <br /> to all individuals throughout the length of this Contract,pursuant to 1-07.11 REQUIREMENTS <br /> FOR NONDISCRIMINATION. The Contractor must comply with all local, state and federal <br /> laws pertaining to non-discrimination and equal employment opportunity. <br /> IThe City of Everett hereby gives public notice that it is the City's policy to assure full <br /> compliance with Title VI of the Civil Rights Act of 1964, the Civil rights Restoration Act of <br /> 1987, and related statutes and regulations in all programs and activities. Title VI requires that <br /> no person shall, on the grounds of race, color, sex, or national origin be excluded from <br /> participation in,be denied the benefits of,or be otherwise subjected to discrimination under any <br /> Federal Aid Highway program or other activity for which the City receives Federal financial <br /> assistance. <br /> I Any person who believes they have been aggrieved by an unlawful discriminatory practice <br /> under Title VI has a right to file a formal complaint with the City of Everett. Any such <br /> complaint shall be in writing and filed with the City's Title VI Coordinator within 180 calendar <br /> I <br /> days following the date of the alleged discriminatory occurrence. Title VI Discrimination <br /> Complaint Forms may be obtained from the Human Resources office at no cost to the <br /> 1 complainant by calling(425)257-8767. <br /> Notification specific to bidders: <br /> All bidders are hereby notified that the City of Everett will affirmatively ensure that in <br /> I any contract entered into pursuant to this Invitation to Bid, minority business enterprises <br /> will be afforded full opportunity to submit bids in response to this invitation and will not <br /> be discriminated against on the grounds of race, color, sex, or national origin in <br /> I consideration for an Award. <br /> Title VI Assurance <br /> a. The Contractor,with regard to the Work performed during the Contract,shall not <br /> I discriminate on the grounds of race,color, sex or national origin in the selection and <br /> retention of subcontractors, including procurement of materials and leases of equipment. <br /> The Contractor shall not participate either directly or indirectly in such discrimination, <br /> I including discrimination in employment practices. <br /> b. In all solicitations either by competitive bidding or negotiations made by the Contractor for <br /> Work to be performed under a subcontract, including procurement of materials or leases of <br /> I equipment,each potential Subcontractor or suppler shall be notified by the Contractor of <br /> the Contractor's obligations under this Contract. <br /> c.
