SCHEDULE A for the CONSTITUENT SERVICES SUITE <br /> 1. Description of Functionality.The Constituent Services Suite is comprised of the following modules and functionality: <br /> a) Advanced Pet Licensing provides the Client with advanced setup functionality within PetPoint's pet <br /> licensing module, allowing further customization related to pet license issuance and fee calculation. <br /> Functionality includes:ability to define customized rules for fees, expiration dates,issuance requirements <br /> (insurance,vaccinations,pet identification,etc.) based on issuer, animal details (type, sterilization status, <br /> etc.), and / or pet licensing jurisdiction. In addition, inventory tag management functionality provides <br /> tracking of the tags associated with pet licenses issued, including separate tracking for multiple resellers. <br /> Advanced Pet Licensing setup applies to PetPoint's existing pet licensing module and the Online Pet <br /> Licensing module. <br /> b) Online Pet Licensing allows the Client to provide a customizable portal on their and/or partners' <br /> website(s)powered by and integrated with PetPoint which allows their customers to purchase,renew, or <br /> cancel pet licenses and update information on licensed pets. Pethealth shall provide an i-frame interface <br /> which will need to be customized by the client through CSS(Cascading Style Sheets)and added to their/ <br /> partners' website(s). In addition, reports within PetPoint will provide information on pet licenses <br /> processed via Online Pet Licensing. <br /> c) Contract Management/Agency Invoicing allows the Client to manage their service contracts with other <br /> organizations(such as municipalities without a housing facility of their own)for providing care,housing, <br /> or medical treatment and batch invoice those other organizations for those services on a per-animal basis. <br /> The module provides separate setup for each such contract,with variable rules that trigger the appropriate <br /> fees to be billed to the contracted organization depending upon the intake & outcome details for each <br /> animal,as well as the pet owner/guardian during reclaim,and even allows deduction of owner/guardian <br /> paid fees from the invoice. <br /> 2. Annual Fee. For the Initial Term of the Supplemental Agreement,the Annual Fee for the Constituent Services Suite is <br /> based on the version of PetPoint in use by the Client,in accordance with the following table: <br /> Version of PetPoint Annual Fee <br /> Lite/Pro $1,500 <br /> Enterprise $1,250 <br /> 3.
