City of Everett Human Needs Grant <br /> January 1 through December 31,2017 <br /> Cocoon House <br /> Cocoon House Street Outreach and Day Center <br /> EXHIBIT A: Program Summary <br /> Cocoon House Street Outreach and U-Turn Day Center serve homeless and at-risk youth, ages 12 through 24 to <br /> help youth and young adults meet basic needs, obtain safe housing, and increase connections to the <br /> community. In Program Year 2017, Cocoon House Outreach and Day Center will serve at least 500 unduplicated <br /> homeless and runaway youth ages 12-24 with approximately 8,000 basic needs items. At least 300 of these 500 <br /> youth will be from Everett. Of the total youth served, 200 will participate in ongoing case management and will <br /> demonstrate progress on housing and self-sufficiency goals. <br /> The immediate goal of Cocoon House's Outreach and Day Center Services is to increase the overall health of <br /> youth and to reduce unhealthy survival practices of runaway and homeless youth.Through repeated contact <br /> and the provision of a continuum of tangible supports and services, staff builds positive,trusting relationships <br /> that empower youth to request and accept increased assistance that ultimately leads to safe and secure housing <br /> off the streets. <br /> Services include mobile and stationary outreach services, case management, meals and food items, basic needs <br /> (including showers, laundry, clothing, and hygiene items), life skills training, on-site health care services, on-site <br /> substance use treatment and prevention, and recreation/pro-social activities. <br /> Measurable Program Outcomes <br /> Youth involved in ongoing case management will work toward these outcomes: <br /> • Positive Housing Status: Youth who engage in long-term case management consistently progress on <br /> goals related to safe housing <br /> o 70%of youth engaged in long-term case management demonstrate consistent progress on <br /> goals related to safe housing <br /> • Case Management: consistently progress on goals related to positive youth development (education, <br /> employment/income, community connections, recovery, health, life skills, etc.) <br /> o 80%of youth engaged in long-term case management demonstrate consistent progress on <br /> goals related to positive youth development. (This represents an increase of 10%over the PY16 <br /> goal.) <br />
