SECTION 00650 <br /> AFFIDAVIT COINING CONFLICTS OF INTEREST <br /> AND NON PRACTICES <br /> SLATE OF Washington ) <br /> ) ss- <br /> COUNT1 OF King ) <br /> The undersigned, being first duly sworn, on oath sates on behalf <br /> of L u m p k i n , Inc . , hereinafter called the Cont c- <br /> tor, as follows: <br /> A. Conflict of Interest <br /> That the Contractor, by entering into this Contract with the Ow n e r <br /> to perrfurw or provide work, services or materials, has thereby covenented, <br /> and by this affidavit does again covenant and assure, that it has no <br /> dirt or indirect pre uniary or pi uPrietary interest, and that he shall <br /> not aczuire any such interest, which conflicts in any manner or degree <br /> with the work, services or rater-i a1 s required to be performed and/or pro- <br /> vided under this Contract and that it shall not e:cpioy any person or agent <br /> having any suth interest. In the event that the Contractor or its agents, <br /> employes or z resentatives hereafter acquires such a conflict of inter- <br /> est, it sh-11 immediately disclose such interest to the. Owner and take <br /> action i- ediately to eliminate the conflict or to withdraw from this Con- <br /> tract, as the .Owner may require. <br /> B. Continent Fcco and Gratuities <br /> That the Contractor, by entering into this Contract with the Ow n e r <br /> to perfoun or provide work, services or materials, has thereby ccvenenta?, <br /> and by this affidavit does again covenent and assure: <br /> 1. That no person or selling agency except bona fide r ioyr-c; or <br /> designated agents or representatives of the Contractor has been e,liployed <br /> or retained to solicit or secure this Contract with an agreement or under- <br /> standing that a commission, percentage, brokerage, or contingent fee would <br /> be paid; and <br /> 2. That no yidtuities, in the form of entertainment, gifts or other- <br /> wise, were offered or given by the Contractor or any member of its agents, <br /> employes or 'representatives, to any official, member or employee of the <br /> Owner or other governmental agency with a view toward securing this <br /> Contract or securing favorable treatment with respect to the awarding or <br /> a-erding, or the raking of any determination with respect to the perfor- <br /> mance of this Contract. <br /> 00650-1 <br />
