CITY OF EVERETT SPECIAL PROVISIONS <br /> f <br /> Payrolls may be submitted on Federal payroll form WH-347, or equivalent. The <br /> reverse side of the form contains an affidavit that shall be filled out and signed. If <br /> the Contractor's payroll reports are computerized, the computerized reports may <br /> be submitted along with a Statement of Compliance affidavit photo copied from <br /> the back of form WH-347, or equivalent. <br /> The first payroll submitted for the Work for both the Contractor and each <br /> Subcontractor shall be labeled "Initial." The last payroll submitted for the Work <br /> for both the Contractor and each Subcontractor shall be labeled "Final." Payrolls <br /> shall be sequentially numbered for all periods in which Work has been done. A <br /> certificate of completion for the Work, signed by the City, will constitute <br /> acceptance of the Work. The issuance of this certificate of completion will not <br /> constitute acceptance of unauthorized or defective Work or material is performed. <br /> 1-07.11 Requirements for Nondiscrimination <br /> Supplement 1-07.11 as follows: <br /> The Contractor will be required to assure that equal employment opportunities will be <br /> in effect to all individuals throughout the length of this Contract, pursuant to 1-07.11 <br /> REQUIREMENTS FOR NONDISCRIMINATION. The Contractor must comply with all <br /> local, state and federal laws pertaining to non-discrimination and equal employment <br /> opportunity. <br /> The City of Everett hereby gives public notice that it is the City's policy to assure full <br /> compliance with Title VI of the Civil Rights Act of 1964, the Civil rights Restoration Act <br /> of 1987, and related statutes and regulations in all programs and activities. Title VI <br /> requires that no person shall, on the grounds of race, color, sex, or national origin be <br /> excluded from participation in, be denied the benefits of, or be otherwise subjected to <br /> discrimination under any Federal Aid Highway program or other activity for which the <br /> City receives Federal financial assistance. <br /> Any person who believes they have been aggrieved by an unlawful discriminatory <br /> practice under Title VI has a right to file a formal complaint with the City of Everett. Any <br /> such complaint shall be in writing and filed with the City's Title VI Coordinator within <br /> 180 calendar days following the date of the alleged discriminatory occurrence. Title VI <br /> Discrimination Complaint Forms may be obtained from the Human Resources office at <br /> no cost to the complainant by calling (425) 257-8767. <br /> Notification specific to bidders: <br /> All bidders are hereby notified that the City of Everett will affirmatively ensure that <br /> in any contract entered into pursuant to this Invitation to Bid, minority business <br /> enterprises will be afforded full opportunity to submit bids in response to this <br /> invitation and will not be discriminated against on the grounds of race, color, sex, <br /> or national origin in consideration for an Award. <br /> Title VI Assurance <br /> a. The Contractor, with regard to the Work performed during the Contract, shall not <br /> discriminate on the grounds of race, color, sex or national origin in the selection <br /> and retention of subcontractors, including procurement of materials and leases of <br /> equipment. The Contractor shall not participate either directly or indirectly in such <br /> discrimination, including discrimination in employment practices. <br /> b.
