City of Everett Human Needs Grant <br /> January 1 through December 31,2017 <br /> Cocoon House <br /> Cocoon House Central: Emergency Teen Shelter <br /> EXHIBIT A: Program Summary <br /> Cocoon House Central Emergency Shelter is an eight(8) bed residential facility located at 2726 Cedar Street in <br /> Everett.This shelter provides housing and supportive services to homeless and at-risk youth ages 12 through 17 <br /> for up to 21 nights. Cocoon House staff work one-on-one with each youth to create and implement an <br /> individualized care plan focusing on stability, securing safe housing, and strengthening the youth's connection to <br /> school. Services include safe housing, basic needs, case management, education and employment assistance, life <br /> skills training, mental health counseling, substance use treatment and prevention, and enrichment and literacy <br /> activities. <br /> The goals of Cocoon House Central's services are to assist youth to permanently leave the streets and to <br /> improve the safety, educational status, and self-sufficiency of homeless youth.This is accomplished through <br /> providing safe housing and supportive services that foster positive youth development and successful transition <br /> to the family home or independent living. <br /> During Program Year 2017, Cocoon House Central will provide 1525 bed nights to 130 unduplicated youth.At <br /> least 50 of the youth served will be from Everett.A bed night includes an overnight stay in the shelter along <br /> with supportive services and basic needs assistance. <br /> Measurable Program Outcomes <br /> Cocoon House prioritizes two (2) outcomes for homeless youth emergency shelter: <br /> • Exit to safe housing with a priority of family reunification: <br /> • 75%of youth served will exit to safe housing <br /> • Engagement in education activities with a priority of school enrollment and academic achievement: <br /> • 70%of youth will have a positive school status at exit <br />
