A. Has been convicted within the last ten years of any felony that directly relates to the teaching position. By way of example only, such felonies might include: crimes against vulnerable persons, such as children, the elderly, or the disabled; crimes of dishonesty; or crimes using, or threatening, violence, including, but not limited to, the use, display or threat of a weapon. B. Has behaved in such a way that the Service Provider reasonably concludes that the proposed teacher does not possess the skill, care and judgment necessary to be a recreation teacher with due regard for the students, facility, or other persons who may be in or near the class. 25. CERTIFICATION AND DOCUMENTATION required as follows: a) ®Yes ❑No Current First Aid/CPR card required for a staff on site during the instruction of these classes. b) Provide Certificate of insurance for the duration of the contract listing City of Everett as additionally insured (#11.A2) If the insurance expires prior to the end of the contract, a new Certificate of Insurance must be received by the Point of Contact by the expiration date of the previous term. c) Provide an Additional Insured Endorsement on the insurance as stated in contract (#11D). d) Provide a UBI number (#16). e) Return two signed and completed original contracts Specific Requirements for Puget Sound Volleyball Association aka USA Volleyball 1. Provide leadership and planning for beach volleyball camps a) All Puget Sound Volleyball staff members will pass background checks conducted by Puget Sound Volleyball (PSV). b) PSV will monitor, track and be responsible for all program volunteers and interns including maintaining on file volunteer applications signed by parents/guardians of participants under the age of 18 that identify the volunteer program as a separate PSV sponsored program. c) PSV will escort camp participants to and from the Jetty on the Jetty Island Ferry. d) PSV will provide clinics with instruction for all registered participants. e) PSV will provide equipment and course syllabus. f) PSV and the lead counselor will be available at all times during camp hours via cell phone provided by PSV or by use of a personal cell phone. g) PSV will designate a lead instructor on site during all camp hours to supervise in the absence of the Head Instructor. h) PSV will monitor and evaluate student's progress. i) PSV will maintain and submit course evaluations and attendance rosters provided by the Point of Conduct after each session; prior to submitting an invoice for payment.
