1 RESOLUTION NO. c lca7 f <br /> 2 <br /> A RESOLUTION authorizing the executions of an addendum amending <br /> 3 the agreement with Dr. George Cozzetto for professional services <br /> as medical advisor for the City's emergency medical services <br /> 4 program. <br /> 5 <br /> 6 WHEREAS, on January 1, 1982, the City of Everett entered into an agreement <br /> 7 with Dr. George Cozzetto as Medical Advisor for the City's emergency medical services <br /> 8 program; and <br /> 9 WHEREAS, since the execution of said agreement, the City has expanded its <br /> 10 emergency medical services program by adding a program providing for a dedicated <br /> 11 paramedic unit; and <br /> 12 WHEREAS, the addition of a paramedic program will require Dr. Cozzetto to <br /> 13 expend considerably greater time and effort in advising the City's emergency medical <br /> 14 services program; <br /> 15 NOW, THEREFORE, be it resolved by the Everett City Council as follows: <br /> 16 <br /> That the Mayor is hereby authorized to execute a contract addendum with <br /> 17 Dr. George Cozzetto which amends his present contract to reflect his <br /> additional duties, increases his rate of compensation, and incorporates the <br /> 18 contract addendum into the original agreement. <br /> 19 <br /> 20 <br /> 21 <br /> 22 Councilman Intrg c ng solution <br /> 23 �j <br /> PASSED and APPROVED this ! day of June, 1982. <br /> 24 <br /> 25 <br /> 26 <br /> Council President <br /> 27 <br /> 28 <br /> 29 <br /> 30 <br /> 31 <br /> 32 <br />
