Welds were excavated and re-welded with <br /> no relevant indications noted. <br /> Visual inspection of fillet welds and bolting per details S3.2/4, 5. The A325 bolts were installed in <br /> accordance with approved specifications. <br /> Note: The following dE)ficiencies were noted at the column on southern end.. <br /> 1. An anchor bolt broke off—contractor repaired by welding in a plate and drilling and <br /> epoxying a new anchor bolt. This was done without prior approval from engineer or special <br /> inspection of epoxy anchor. <br /> 2. Top beam-to-cclumn connection was altered due to hole alignment. Per engineer email <br /> dated 04/04/16 the column cap plate and bottom of beam flange should have received a <br /> 1/4"fillet weld located at north and south edges.
