Joe DapceviCh <br /> ..........------------_--....--.................... ....... <br /> - .....-- ----- —..---------.._ _—..—. <br /> Attachments: --- <br /> Site Plan(s) ❑ Density Test Summary Signature: ' <br /> ❑ Installation Records ❑ Other Reviewed By: <br /> PURPOSE of VISIT: GeoDesign representative, Joe Dapcevich, was on-site part time at the request of John Feltner with <br /> Polygon to observe the over-excavated subgrade backfill for Lots 135 and 140. <br /> SUMMARY of OBSERVATIONS: <br /> of 135 140: <br /> GeoDesign noted that the exposed footing subgrade for Lot 135 &140 was covered in approximately 3 to 6 inches of <br /> surficially saturated material,which we recommended be removed prior to our footing subgrade evaluation. GeoDesign <br /> observed that the footing subgrade that had been over-excavated and evaluated in the previous site visit had been lined <br /> with a non-woven geotextile fabric. Excavations were then backfilled with 2-inch clean crushed railroad ballast material <br /> and compacted into a firm and unyielding mass using the bucket of an excavator. <br /> This report presents opinions formed as a result of our observation of activities relating to geotechnical engineering or environmental services.We rely on the contractor to comply with the plans and <br /> specifications throughout the duration of the project irrespective ofthe presence of our representative. Dur work does not include supervision or direction of the contractor,the contractor's employees <br /> or agents.
