The City will offer insurance plans to all Appointive Officers comprised <br /> of the following on terms and conditions determined by the City: <br /> 1. Basic and major medical,vision,and dental coverage for the Appointive <br /> Officer and eligible dependents,provided the full-time officer makes a 10 <br /> percent contribution toward the cost of the medical coverage. Part-time <br /> officers pay a pro-rated contribution toward their medical coverage,based <br /> on their hours worked during a pay period. If an officer chooses a carrier <br /> other than the City's self-insured medical insurance program,the officer <br /> shall pay the difference in premiums. <br /> 2. The face amount of life insurance and the principal sum of accidental <br /> death and dismemberment insurance for an Appointive Officer shall be <br /> equal to two times the Appointive Officer's basic annual earnings, <br /> rounded to the next higher multiple of$1,000,if not already a multiple of <br /> $1,000,plus$2,000. The maximum face amount and maximum principal <br /> sum shall each be$250,000. <br /> 3. Long-term disability insurance shall be 66 2/3%up to $7,500 of the <br /> • Appointive Officer's pre-disability monthly earnings,subject to policy <br /> conditions and limitations. <br /> 4. Voluntary dependent life insurance covered at the Appointive Officer's <br /> expense. <br /> 5. Annual physical examinations for all Department Heads. <br /> Section 2. Ordinance No. 1594-89 § 3.A.,as amended by Ordinance No. 2560-01 § 3 <br /> and 4,and Ordinance No. 3295-12 § 3 (Chapter 234.08 <br /> reads as follows: <br /> A.
