ILSuite 117 Woodridge, 60517 Phone:(630)-985-1300 Fax.(630)-985-1310 SCHEDULE B— Extension / Request for Additional Services Relationship To The Original Contract and Purpose: This is an addendum to the existing WebQA Services Purchase Agreement executed between WebQA Incorporated and the city of Everett,WA("Customer")signed 10/5/2016 with its current term starting 10/1/2016 and ending 9/30/2017("Current Contract Period"). The purpose of this Addendum is to add the new services selected below. Additional Services: Customer is ordering the following services at the following prices. Subscribed Services GovQA State of WA Public Provides specific State of WA reporting module to comply with House Bill 1594 reporting requirements. Records Reporting Module Include associated means for tracking and reporting to include: • Average Time to First Response • %Requests Closed Within 5 Days • %Requests with Estimate>5 Days • Estimated Average Time to Close • %Requests with Estimates Sent • Actual Time to Close • Requests Seeking additional Clarification • Requests Denied with Reason Count • Requests Abandoned • Total Request by Type of Requestor • %Requests Fulfilled Electronically • %Requests Fulfilled by both Electronic and Physical Records • Requests requiring Scanning of Physical Records • Estimated Agency Staff Time Spent on Request • Total Staff Time Costs Incurred • Average Cost of Staff Time per Request • Invoice Payments Received(Must Use GovQA Invoicing) GovQA Invoicing Module The Invoicing Module will allow you to create cost estimate letters to be sent via email,portal and mail to a requester. The Cost Estimate can define a deposit due. A Cost Estimate can easily be converted to an Invoice. The Invoice can track partial payments and we notify staff when a requester has outstanding invoices prior to staff beginning a new request. Requesters will be flagged with this information and work flow can be applied to support your business process. This module can be connected to a payment module to allow the requester to make payments online. Software Subscription Costs Software License(s) Annual Cost GovQA State of WA Public Records Reporting Module $3,000 GovQA Invoicing Module $2,400 Total $5,400 Software Subscription Implementation Costs(One-Time) GovQA State of WA Public Records Reporting Module Waived GovQA Invoicing Module $1,000 Total $1,000 Start Date and Timeframes: The START DATE for the term of this agreement will be 9/1/2017 and go to a SYNC DATE of 10/1/2017 at which point the term for these Additional Services will sync to the term of the Original Contract and all subsequent renewals. Billing: START DATE(9/1/2017)to SYNC DATE(10/1/2017)FEE WILL BE WAIVED. Billing will occur on the SYNC DATE of 10/1/2017 and include the annual fees and implementation fee associated with the Additional Services described above.
