• MAP and Work Crew participants who are not eligible for Medicaid and/or do not meet Division <br /> of Behavioral Health and Recovery(DBHR)Access to Caro Standards. <br /> This will further enhance the MAP and Work Crew programs through effective use of relevant <br /> community resources. <br /> "Life Cycle" of a Client <br /> 1. Referral from MAP Liaison or Work Crew Program Manager <br /> 2. Meet with Employment Specialist—review referral, goals, barriers,needs,frequency <br /> of meetings, etc. <br /> 3. Create Goal Plan--no need for mental health assessment <br /> a. Includes vocational assessment <br /> b. Goals are specific to the individual--"meet the individual where they are at" <br /> c. Collaboration between Employment Specialist and natural supports,referring <br /> party,mental health provider, and with employer if individual wishes <br /> d. Could include follow-along services,resume writing,mock interviews, <br /> education around hygiene,dress,determining what/if accommodations are <br /> needed/helpful <br /> 4. Every meeting with individual is related to one or more of the listed goals <br /> 5. Goal plan is updated as needs/wants change <br /> 6. List Target dates for each goal <br /> 7. Entire process: 642 months, including support after individual is employed, if needed <br /> Timeline and Staffing <br /> One FTE Employment Specialist <br /> • Experience working with difficult populations, barriers,and slow progress <br /> • Bachelor's degree in social service-related field <br /> • Experience with identifying'mental illness/substance abuse in order to discuss behavioral <br /> health education and possible tax incentives with potential employers <br /> • Familiarity with,or ability learn quickly the IPS model, supported employment <br /> • May not need to meet qualifications for Agency Affiliated Registration if NSBHO is not <br /> billed <br /> Contingent upon funding,we anticipate it would take three months to hire and train staff. <br /> The Employment Specialist could be sited either at Bridgeways, or in the community, at a <br /> "community services hub," or a location otherwise convenient to MAP and Work Crew <br /> participants. <br /> Anticipated Outcomes <br /> We anticipate serving 15 individuals referred via the MAP and Work Crew programs during <br /> Year One,building to 25 participants during Year Two,as the caseload matures. <br /> Experience with our Supported Employment Program indicates about 40%will achieve <br /> employment in the community, <br /> Additional positive participant outcomes could include: <br /> • Increased stability <br /> • Increased self esteem <br /> b <br /> 1 <br /> l <br />
