• <br /> Exhibit "A" <br /> Scope of Services, Time of Performance, Budget, <br /> Payment, Notices, and Representatives <br /> I. Scope of Service <br /> A. Activities/Project Description <br /> The Subgrantee will be responsible for administering the Adult Dental Care for the Uninsured <br /> program in a manner satisfactory to the City and consistent with any standards required as a <br /> condition of providing these funds. Such program will include the following activities eligible under <br /> the Community Development Block Grant Program: <br /> Program Delivery <br /> Activity: <br /> Community Health Center of Snohomish County(CHC) provides primary medical and dental care for <br /> low-income residents living in Snohomish County. The funds under this grant proposal will provide <br /> dental care for low-income, uninsured adults living in the City of Everett. Dental services include <br /> assessment, cleaning, fillings and extractions. Patients without insurance are treated based on a <br /> sliding fee schedule that discounts payment for services based on family size and income. <br /> Homeless individuals are not charged for their care and no one is denied services because of <br /> inability to pay. <br /> Everett residents who come to CHC for services are provided care according to their needs. CHC <br /> clinicians work with patients to assess their condition and provide appropriate treatment. Dentists <br /> and dental hygienists provide dental care. Behavioral health specialists, nutritionists, clinical <br /> assistants, interpreters, receptionists and administrative staff support the providers. <br /> Services provided to patients under this grant include: <br /> • Dental care— diagnosis and treatment— for uninsured adults. <br /> One service unit is considered equal to one office visit. Each patient under this program will receive <br /> 2 office visits (initial to determine needs, second to do procedure) generally. CHC will use CDBG <br /> grant funds to serve uninsured, low-income adults living in the City of Everett. <br /> CHC operates facilities in north and south Everett. The clinic in north Everett is located on 14th & <br /> Broadway and the clinic in south Everett is located on 112th Street SW. CHC dental clinics are <br /> staffed with dentists, dental hygienists, dental assistants, and receptionists. A pharmacy, that serves <br /> CHC patients, is located at each medical clinic. Behavioral health Specialists are located at each <br /> clinic to assist providers in identifying and addressing behavioral health issues underlying health <br /> conditions. <br /> A-1 <br />
