D. Levels of Accomplishment - Goals and Performance Measures <br /> In addition to the normal administrative services required as part of this Agreement, the Subgrantee <br /> agrees to provide the following levels of program services: <br /> Activity Units of Service* per Month Total Units/Year <br /> Calls 29 350 <br /> Total People or Households to be served Total unduplicated Everett Residents or <br /> Households to be served <br /> 700 350 (1st party) & 350 (2nd party) <br /> *Note: Provide definition of Units of Service, which may include such measures as units rehabbed, <br /> persons or households assisted, or meals served, here. If this is a construction project, please <br /> explain here how the improvements will be used and how they benefit the community, what the <br /> target population is for the project, etc. <br /> The Subgrantee agrees to track outcome performance measures as required by the U.S. <br /> Department of Housing and Urban Development and the City. The Subgrantee will report the <br /> outcomes to the City on at least a quarterly basis. Information and forms will be provided to the <br /> Subgrantee by the City for reporting this information. <br /> E. Coordinates program <br /> activities including training and mediation services. <br /> Practicum Coordinator,Alisha Robinson, Professional Mediator. Coordinates mediators, activities and <br /> qualifications of the 116 volunteer mediators who serve the center. <br /> Housing Program Manager, Galina Volchkova, BA-Journalism. 16 years program coordination <br /> experience and supporting low-income households and homeless. <br /> Multi-Cultural Services Manager,Anahi Machiavelli, JD, BA. Coordinates all Spanish speaking services <br /> and provides outreach to Spanish speaking community. <br /> Fair Housing Specialist,James Rankin, AA Human Services. Provides Fair Housing Counseling and <br /> Training to the community. <br /> Landlord/Tenant Specialist Spanish, Ilse Anahi Garcia, BA Psychology, supports Fair Housing Team <br /> with Spanish Speaking clients and teaches classes in Spanish. <br /> Volunteers: 116 Volunteer Certified Professional Mediators, Presenters and Trainers. <br /> A-3 <br />
