Exhibit "A" <br /> Scope of Services, Time of Performance, Budget, <br /> Payment, Notices, and Representatives <br /> I. Scope of Service <br /> A. Activities/Project Description <br /> The Subgrantee will be responsible for administering the Operation School Bell program in a manner <br /> satisfactory to the City and consistent with any standards required as a condition of providing these <br /> funds. Such program will include the following activities eligible under the Community Development <br /> Block Grant Program: <br /> Program Delivery <br /> Activity: <br /> All services will be carried out in our facility at 5107 Evergreen Way in Everett by trained Assistance <br /> League of Everett Volunteers. Our Operation School Bell Chairman for this year is Rock Dorsey. <br /> Ms. Dorsey supervises the activities of the volunteers working directly with the program. Each year a <br /> total of about 100 volunteers participate in Operation School Bell program delivery. <br /> The in-facility Operation School Bell program serves younger children and is based in our <br /> headquarters on Evergreen Way. Children are transported to our facility by their schools to receive <br /> their clothing. From our supply of new school clothing purchased specifically for the program, <br /> children select three outfits of school clothes, a warm winter jacket, and a week's supply of <br /> underwear and socks. In addition, they are provided with a hygiene kit with grooming supplies. For <br /> those with greatest need, we provide a $25 voucher for new shoes for smaller children and a $30 <br /> shoe voucher for older boys and girls, and a $15 shoe voucher for the ECEAP children. Cost for the <br /> service averages about $92.60 per child depending on the size of clothing, and this includes the <br /> shoe voucher. We anticipate that we will serve approximately 3,800 children countywide in our <br /> Everett facility during the period of August 1st 2017 to May 31St 2018. In this period, the $10,000 <br /> provided by the City of Everett will serve approximately 107 children, pre-school to eighth grade, <br /> living within the City of Everett. <br /> General Administration <br /> (Add description of general administrative services to be performed in support of activities noted above) <br /> Assistance League of Everett has no paid staff. However, the skill, dedication, and <br /> professionalism of our volunteers are the equal of organizations with a full roster of paid <br /> employees. As stated above, our Operation School Bell Chairman for this year is Rock Dorsey. <br /> Ms. Dorsey is responsible for the overall direction, scheduling, and supervision of the many ALE <br /> volunteers who work in the program. Because we have no paid staff, our Board President, Carla <br /> Hogan, fills the role that the Executive Director would play in other non-profit organization with paid <br /> staff. Our governing board also includes a Vice President of Philanthropic Programs. This year <br /> that position is held by Phyllis Busch, another long-time ALE volunteer.
