D. Levels of Accomplishment- Goals and Performance Measures <br /> In addition to the normal administrative services required as part of this Agreement, the Subgrantee <br /> agrees to provide the following levels of program services: <br /> Activity Units of Service* per Month Total Units/Year <br /> New school clothing for <br /> 26 units of service per <br /> low-income Everett month for seven (7) 107 low-income children <br /> children in grades months. preschool to grade 8 . <br /> preschool to grade 8. <br /> Total People or Households to be served Total unduplicated Everett Residents or <br /> Households to be served <br /> 107 low-income children preschool to 107 low-income children preschool to <br /> grade 8. grade 8 . <br /> *Note: Provide definition of Units of Service, which may include such measures as units rehabbed, <br /> persons or households assisted, or meals served, here. If this is a construction project, please <br /> explain here how the improvements will be used and how they benefit the community, what the <br /> target population is for the project, etc. <br /> The Subgrantee agrees to track outcome performance measures as required by the U.S. <br /> Department of Housing and Urban Development and the City. The Subgrantee will report the <br /> outcomes to the City on at least a quarterly basis. Information and forms will be provided to the <br /> Subgrantee by the City for reporting this information. <br /> E. Staffing <br /> [Add Provide a list of staff and time commitments to be allocated to the activity specified in I.A. above] <br /> ALE has no paid staff. All of our programs are planned, managed, staffed and evaluated by trained <br /> volunteers. No City of Everett funds will be used for staff salaries or staffing costs of any kind. Every <br /> cent of the $10,000 awarded to the Operation School Bell program will be expended for new school <br /> clothing and new shoes for low-income children living within the City of Everett. <br /> Operation School Bell is our largest program and our chairman, Rock Dorsey, will work the <br /> equivalent of a full-time schedule during the period when the program is most active—August 2017 <br /> to March/April 2018. The number of volunteers working on the program is so large that it would be <br /> impractical to list all of the hours worked by each volunteer and the tasks performed. Each year <br /> more than 100 ALE volunteers work on some aspect of Operation School Bell, contributing about <br /> 20,000 service hours. However, it is important to reiterate that no CDBG funds will be used for staff <br /> costs, overhead or any other expense. All of the funds will be used to purchase new clothing and <br /> shoes for the boys and girls. <br /> A-3 <br />
