Exhibit "A" <br /> Scope of Services, Time of Performance, Budget, <br /> Payment, Notices, and Representatives <br /> I. Scope of Service <br /> A. Activities/Project Description <br /> The Subgrantee will be responsible for administering the Meals on Wheels program in a manner <br /> satisfactory to the City and consistent with any standards required as a condition of providing these <br /> funds. Such program will include the following activities eligible under the Community Development <br /> Block Grant Program: <br /> Program Delivery <br /> Good nutrition and socialization are essential to the health and quality of life for older adults and <br /> people with disabilities. Older adults and people with disabilities who cannot prepare their own meals <br /> or have to choose between food and other basic needs are at greater risk of hospitalization, chronic <br /> disease, more costly long term care, and displacement from their home. To address this need, <br /> Homage Senior Services' Meals on Wheels program provides nutritious meals for homebound older <br /> adults and people with disabilities. <br /> Meals on Wheels recipients receive weekly deliveries of 7-14 meals, which they select from a menu <br /> of 25 meals. Recipients also receive nutrition assessments, counseling, and nutrition education <br /> tailored to their individual needs. This $11,830 grant award will provide 1,971 meals to 20 individuals <br /> during the contract period. <br /> General Administration <br /> Senior Services of Snohomish County's general administrative functions include Executive, Finance, <br /> Human Resources, and Information Technology. <br /> B. National Objectives <br /> The Subgrantee certifies that the activities carried out with funds provided under this Agreement will <br /> meet one or more of the following CDBG National Objectives - 1) benefit low/moderate income <br /> persons, 2) aid in the prevention or elimination of slums or blight or, 3) meet community development <br /> needs having a particular urgency - as defined in 24 CFR Part 570.208. Briefly describe how this <br /> National Objective will be met. <br /> C. Performance Monitoring <br /> A-1 <br />
