Exhibit "A" Scope of Services, Time of Performance, Budget, Payment, Notices, and Representatives I. Scope of Service A. Activities/Project Description The Subgrantee will be responsible for administering the Meals on Wheels program in a manner satisfactory to the City and consistent with any standards required as a condition of providing these funds. Such program will include the following activities eligible under the Community Development Block Grant Program: Program Delivery Good nutrition and socialization are essential to the health and quality of life for older adults and people with disabilities. Older adults and people with disabilities who cannot prepare their own meals or have to choose between food and other basic needs are at greater risk of hospitalization, chronic disease, more costly long term care, and displacement from their home. To address this need, Homage Senior Services' Meals on Wheels program provides nutritious meals for homebound older adults and people with disabilities. Meals on Wheels recipients receive weekly deliveries of 7-14 meals, which they select from a menu of 25 meals. Recipients also receive nutrition assessments, counseling, and nutrition education tailored to their individual needs. This $11,830 grant award will provide 1,971 meals to 20 individuals during the contract period. General Administration Senior Services of Snohomish County's general administrative functions include Executive, Finance, Human Resources, and Information Technology. B. National Objectives The Subgrantee certifies that the activities carried out with funds provided under this Agreement will meet one or more of the following CDBG National Objectives - 1) benefit low/moderate income persons, 2) aid in the prevention or elimination of slums or blight or, 3) meet community development needs having a particular urgency - as defined in 24 CFR Part 570.208.
