Page 1 of 5 <br /> EXHIBIT A <br /> Work Order Form <br /> ENVIRONMENTAL SERVICES PROJECT SCOPE AND ESTIMATE <br /> Project Title: RI/FS and CAP— Former Smith Street Sawmill W.O. #1 <br /> 1. Project Scope: <br /> CONSULTANT will conduct a Remedial Investigation (RI), Feasibility Study(FS) and Cleanup Action Plan (CAP) <br /> for the approximate 4.7-acre Former Smith Street Sawmill property located at 3600 Smith Avenue(Snohomish <br /> County Tax Parcel Numbers: 00-5760-019-000-05, 00-5760-019-000-06, 00-5760-043-000-02, 00-5760- <br /> 012-000-03, 29-0529-003-008-00, 29-0529-002-012-00, 29-0529-003-030-00 and 29-0529-003-010-00) <br /> in Everett, Washington. We understand that the City has purchased the property and plans to conduct <br /> assessment and cleanup of the Property in accordance with the Model Toxics Control Act(MICA, Washington <br /> Administrative Code (WAC) 173-340). Please refer to GeoEngineers' Phase I and Draft Phase II ESA reports <br /> dated October 27,2016 and January 3, 2017, respectively,for a description of the Property and the results of <br /> site assessments completed to date. We further understand that the City desires to enter into Ecology's <br /> Voluntary Cleanup Program(VCP)and to conduct the cleanup with Ecology review and Opinion,with the desired <br /> end result of a "No Further Action" determination for the Property. <br /> The proposed scope of work includes an RI Work Plan to complete borings and install monitoring wells to <br /> obtain soil and groundwater samples sufficient to complete an RI/FS and CAP in accordance with Ecology's <br /> expectations for VCP sites http://www.ecy.wa.gov/programs/tcp/vcp/Vcpmain.htm. We recommend <br /> submitting a VCP application and an RI Work Plan for Ecology review and Opinion under the VCP, and <br /> subsequently submitting the draft RI/FS and CAP to Ecology for a VCP Opinion on the proposed cleanup. <br /> Ecology's current timetable for VCP review and Opinion is 90 days following assignment of the Site to an <br /> Ecology Site Manager. Ecology's Northwest Regional Office VCP wait list of sites awaiting assignment to a Site <br /> Manager, is in the range of about 6 months. <br /> Bid specifications for cleanup, and performance and compliance monitoring of the cleanup will be covered in <br /> a subsequent authorization. <br /> Tasks associated with the RI/FS and CAP are as follows: <br /> Task 1:Project Kickoff and Remedial Investigation Work Plan <br /> 1. Meet with the City for a project kickoff to discuss project background, project expectations, current site <br /> conditions,site redevelopment plans and overall cleanup timeline. <br /> 2. Identify site characterization data gaps that need to be filled to complete an RI. Prepare a draft remedial <br /> investigation(RI)Work Plan that outlines the goals of the investigation and summarizes field investigation <br /> procedures, exploration locations and the analytical testing program for the RI. <br /> 3. Submit the draft RI Work Plan to Ecology with an application for the City's entry into the Voluntary Cleanup <br /> Program requesting an opinion on the Work Plan. Finalize RI Work Plan following receipt of comments <br /> from Ecology. <br /> 4.
