A. Has been convicted within the last ten years of any felony that directly <br /> relates to the teaching position. By way of example only, such felonies <br /> might include: crimes against vulnerable persons, such as children, the <br /> elderly, or the disabled; crimes of dishonesty; or crimes using, or <br /> threatening, violence, including, but not limited to, the use, display or threat <br /> of a weapon. <br /> B. Has behaved in such a way that the Service Provider reasonably concludes <br /> that the proposed teacher does not possess the skill, care and judgment <br /> necessary to be a recreation teacher with due regard for the students, <br /> facility, or other persons who may be in or near the class. <br /> 25. CERTIFICATION AND DOCUMENTATION required as follows: <br /> a) ®Yes No Current First Aid/CPR card required for a staff on site <br /> during the instruction of these classes. <br /> b) Provide Certificate of insurance for the duration of the contract listing <br /> City of Everett as additionally insured (#11.A2) If the insurance expires <br /> prior to the end of the contract, a new Certificate of Insurance must be <br /> received by the Point of Contact by the expiration date of the previous <br /> term. <br /> c) Provide an Additional Insured Endorsement on the insurance as <br /> stated in contract (#1 1 D). <br /> d) Provide a UBI number (#16). <br /> e) Return two signed and completed original contracts <br /> Specific Requirements for Camp Fire Snohomish County <br /> 1. Provide the following for all classes taught <br /> a) This Professional Services Agreement is effective when the provider receives a <br /> written "Notice To Proceed"with a specific date from the provider's Point of Contact. <br /> b) Plan and implement Self-Reliance classes for children 7-14 years old <br /> c) Maintain attendance records <br /> d) Assist with promotion of classes by distributing flyers. <br /> e) Monitor and evaluate student progress. <br /> f) Maintain and submit course evaluations. <br /> g) Monitors students and activity for safety. <br /> h) Prepare class materials and handouts for each class <br /> i) Renew Annual City of Everett Business License for 2018, no later than January 31, <br /> 2018 <br /> 2. Dates and Times of Program Currently Scheduled <br /> Class Dates Day Time Ages Min/Max Location <br /> Baby Sitting Basics Feb 10-17 Sa 9am- 10-14 8/15 Preschool Room <br /> 1pm <br /> 3. Plus additional classes/programs as agreed upon by Program Supervisor and Service <br /> Provider as detailed in the Quarterly Program Guide. <br /> 4. The City retains the right to cancel any program that does not meet minimum registration <br /> requirements. <br />
