c) Statement that no personal vehicle is used in fulfilling this Agreement as stated in <br /> Contact Page 3 Paragraph 9. <br /> d) Provide a current Driver's Abstract <br /> e) Renew annual City of Everett Business License for 2018 no later than January <br /> 31, 2018. <br /> f) Provide Certificate of insurance for the duration of the contract listing City of <br /> Everett as additionally insured. If the insurance expires prior to the end of the <br /> contract, a new Certificate of Insurance must be received by the Point of Contact <br /> by the expiration date of the previous term. <br /> g) Provide an Additional Insured Endorsement on the insurance as stated in <br /> Specific Requirements 2b. <br /> 4. Provide leadership and planning for outdoor activities <br /> a) Includes: hikes, tours, skiing, snowshoeing, and biking. <br /> a) Develop trips: planning dates, times, routes, and best activities. <br /> b) Provide equipment lists and agendas as needed. <br /> c) Lead pre-trip planning session with participants. <br /> d) Provide trip evaluation and report after each trip. <br /> e) Develop curriculum to instruct outdoor activities. <br /> f) Monitor and evaluate students' progress. <br /> g) Maintain and submit course evaluations. <br /> h) Maintain course attendance sheets. <br /> i) Monitors students and activity for safety. <br /> j) Compile annual trip summary. <br /> k) Write promotional information for activities <br /> 5. Dates and times (see attached outdoor trips and classes) <br /> Hourly rate for each trip as an activity leader as referenced in #4 will be based on the <br /> following. Actual trip leading time plus approx. 1.5 hours/trip set up /clean up. Additional <br /> hours with supervisor approval to assist with trip planning. Classes to include hiking, <br /> snowshoeing, skiing, walking, trips and tours and other instruction as requested. <br /> Overnight trip compensation will not exceed twelve (12) hours per day. <br /> 6. Plus additional classes/programs as agreed upon by Program Supervisor and <br /> Service Provider, and as detailed in the quarterly Program Guides. <br /> 7. The City retains the right to cancel classes that fall below the minimum number of <br /> students. <br />
