IL <br /> residents may be resistant to participating in services,therefore, staff work to engage and <br /> develop relationships with residents so that over time residents may more readily choose to <br /> participate in services. Building community among residents is essential to this support services <br /> model. When residents experience their living environment as their home they become more <br /> invested in contributing to a safe and quality place to live. The CCS and partner agency staff of <br /> this project work as a team to provide an array of services to residents. <br /> ■ DETAIL OF SUPPORTIVE SERVICES <br /> Each new resident will be matched with a Housing Case Manager from the most appropriate <br /> partner agency based on the resident's needs. These Housing Case Managers will meet with <br /> residents to identify their supportive service needs,provide case management services, crisis <br /> intervention,eviction prevention, advocacy and linkages to community resources, and encourage <br /> participation in meaningful activities.Residents are assisted in obtaining and maintaining <br /> fmancial disability benefits such as SSI, SSDI and VA benefits,and are also assisted with <br /> obtaining Medicaid,Medicare, and Veterans medical benefits. Referrals for residents are made <br /> to chemical dependency treatment and/or mental health services through Catholic Community <br /> Services and other resources in the community. Residents needing additional help with personal <br /> care and unit up-keep are referred for chore service.Health care is a primary need of residents at <br /> Everett Supportive Housing. Residents are referred,transported, and accompanied when <br /> necessary to community health clinics in order to establish primary care physicians and/or <br /> visiting nurses may use a private room available in the facility for such purposes. This approach <br /> encourages residents to develop a plan of action to address their physical and mental health care <br /> needs. The goal of services is for residents to obtain and maintain fmancial and medical benefits, <br /> decrease in the use of emergency medical services, establish a relationship with a primary health <br /> care provider,and increase a resident's ability to abide by lease requirements despite a disabling <br /> condition. <br /> The Business Manager aids residents with obtaining documentation for annual income re- <br /> certifications and assist residents with concerns or problems with their income as it relates to <br /> paying their rent. For residents with payees,the Business Manager maintains regular contact <br /> with the payee and informs the payee of any fmancial concerns regarding residents housing. <br /> The Front Desk Staff provide information and referrals to residents,respond to medical <br /> emergencies,intervene in conflict situations, and ensure the safety of the residents' living <br /> environment. A monthly house meeting is convened by the Program Director to discuss <br /> community issues and concerns and encourage active involvement by residents in their <br /> community. These services address primary issues of safety and isolation faced by persons who <br /> have been homeless. <br /> Services by visiting professionals such as case managers,visiting nurses,mental health <br /> counselors, and chemical dependency counselors are coordinated and supported by the Housing <br /> Case Managers. <br /> 24 <br />
