The City Clerk shall issue the license promptly upon <br /> receipt of the application, the information required <br /> 2 to be in the application, the investigation by the <br /> Police Department in accordance with Section 1(G) (5) , <br /> 3 and the license fee. <br /> 4 4. If any person or entity acquires, subsequent to the <br /> issuance of a panoram premise license, a substantial <br /> 5 interest, as defined in Section 1(G) (1) (c) , in the licensed <br /> premises, notice of such acquisition shall be provided in <br /> 6 writing to the City Clerk forthwith. The information <br /> required to be provided pursuant to this section shall be <br /> 7 that information required pursuant to Section 1(G) (1) (c) of <br /> this chapter. <br /> 8 <br /> 5. The Clerk, upon presentation of such panoram premise, <br /> 9 panoram device and manager's applications and before acting <br /> upon the same, shall refer such applications to the Police <br /> 10 Department, which shall make a full investigation as to the <br /> truth of the statements contained therein. <br /> 11 <br /> 12 be and the same is hereby amended to read as follows: <br /> 13 <br /> G. License Application - Report by City Departments. <br /> 14 <br /> 1. Any person seeking a panoram premise, panoram manager's or <br /> 15 panoram device license shall file a written application <br /> with the Clerk on a form provided by the Clerk for that <br /> 16 purpose. All applications for a panoram premise license <br /> shall be submitted in the name of the person or entity <br /> 17 proposing to conduct such business and shall be signed by <br /> such person or his agent and notarized or certified as true <br /> 18 under penalty of perjury. All applications for a panoram <br /> premise license shall be submitted on a form supplied by <br /> 19 the Clerk, which form shall require the following <br /> information: <br /> 20 <br /> a. The name, home address, home telephone number, date <br /> 21 and place of birth, and social security number of the <br /> applicant if the applicant is an individual. <br /> 22 <br /> b. The business name, address and telephone number of <br /> 23 the establishment. <br /> 24 c. The names, addresses, telephone numbers, and social <br /> security numbers of any partners or corporate <br /> 25 officers. <br /> 26 d. The name, address and telephone number of the owner <br /> of the property on which the panoram premise is <br /> 27 located. <br /> 28 2. All applications for a panoram device license shall be <br /> submitted on a form supplied by the Clerk and shall be <br /> 29 submitted in the name of the person or entity owning the <br /> panoram devices and shall be signed by such person or his <br /> 30 agent and notarized or certified as true under penalty of <br /> perjury.
