D. Levels of Accomplishment - Goals and Performance Measures In addition to the normal administrative services required as part of this Agreement, the Subgrantee agrees to provide the following levels of program services: The Boys and Girls Club will provide recreational activities and meals for at least 200 youth, the majority of which are low-income, during the first year following the completion of this project. A forgivable loan, as described in "Exhibit C" of this agreement will provide that the building be used to serve low-income youth for 10 years . *Note: Provide definition of Units of Service, which may include such measures as units rehabbed, persons or households assisted, or meals served, here. If this is a construction project, please explain here how the improvements will be used and how they benefit the community, what the target population is for the project, etc. The Subgrantee agrees to track outcome performance measures as required by the U.S. Department of Housing and Urban Development and the City. The Subgrantee will report the outcomes to the City on at least a quarterly basis. Information and forms will be provided to the Subgrantee by the City for reporting this information. E. Staffing A contractor will be hired to complete the improvements . The City will monitor the progress of the project. II. Time of Performance Services of the Subgrantee shall start on the 15th day of November, 2017 and end on the 31st day of December, 2018. The term of this Agreement and the provisions herein shall be extended to cover any additional time period during which the Subgrantee remains in control of CDBG funds or other assets, including program income. The City may, at its discretion, extend the term of this Agreement to allow for the expenditure of unexpended funds. III. Budget Line Item: Amount: Rehabilitation/Improvements $8, 232 . 00 A-2
