CITY OF EVERETT PUBLIC WORKS 00 43 36 <br /> SPECIAL PROVISIONS PROPOSED SUBCONTRACTOR'S FORM <br /> DOCUMENT 00 43 36—PROPOSED SUBCONTRACTOR'S FORM <br /> 1.1 GENERAL <br /> A. For heating, ventilation, air conditioning, plumbing (as defined by RCW Chap. 18.106) and <br /> electrical work (as defined by RCW Chap. 19.28), Bidder MUST either identify itself or <br /> Subcontractors in the chart below. If Bidder believes such work is not part of the Work, Bidder <br /> ' shall write"NO WORK". <br /> B. Bidder shall not list more than one Subcontractor for each category of Work identified, unless <br /> Subcontractors vary with Bid alternates, in which case the Bidder must indicate which <br /> Subcontractor will be used for which alternate. <br /> ' C. Bidder's Bid shall be deemed non-responsive and void if: <br /> 1. For heating, ventilation, air conditioning, plumbing and electrical work, Bidder fails: (1) <br /> to submit as part of the Bid the names of such Subcontractors; (2) to name itself to <br /> perform such Work; or(3)to write"No Work"; or <br /> 1 2. Bidder names two or more Subcontractors to perform the same Work. <br /> D.
