num
„` PUBLIC WORKS
dent vib e-mail
7
April 7, 2015
Mr. Stoyan Mitrachkov
2301 Colby Ave
Everett, WA 98201
Owner: Mr. Stoyan Mitrachkov
Project: Restore Two Residential Unit and Offices
Job Address: 2301 Colby Ave
Permit No: BW1501-002; Code Citation: CE14-0496
RE: Request for Temporary Certificate of Occupancy for Second Floor Residential Use
Dear Mr. Mitrachkov,
We will consider your request for a temporary certificate of occupancy of the second floor. This
will require approved inspections of the drawings issued for permit on 3/4/2015 (including city-
stamped revisions) per an approved phasing plan for work to be deferred or completed on the
first and second floor plans. This is the minimum requirement to perform inspections. Once
issued, the temporary certificate of occupancy (TCO) applies for a limited six month duration.
1. The phasing plans should identify the deferred area of work, noted as: "Access for
maintenance only, not for occupancy".
a. The phasing plans should be identical to the issued permit drawings, with city
approved modifications.
b. All plumbing fixtures in the deferred work area to be noted as: "Fixtures removed
with water and waste lines capped".
2. A new fire detection system in the deferred area of work is required to alert second floor
residents, per AFM Venturo.
a. First floor plan to show new heat detectors. Second floor plan to show hard-
wired smoke detectors in hallways linked to the monitored system and first floor
heat detectors, with notification devices. Call 425-257-8124 to schedule a field
review.
Electrical
1. The phasing plans should indicate that all electrical wiring or equipment in the building
must be exposed for inspection, and that new electrical work or repair to electrical work
must comply with the 2014 NEC.
Sincerely,
Meg Hley, NA
Plan R-view 4rchitect, 425-257-881
cc: AFM Jim Venturo, BO Tony Lee, Kevin Fagerstrom, Ted Budden, Steve Ingalsbe
Page 1
1
