Joe Dapcevich <br />. ........ .. <br />m Site Plans) <br />❑ Density Test Summary <br />.. . <br />Signature <br />Attachments _.. ..... ...... <br />❑ Installation Records <br />❑ Other <br />_ <br />Reviewed Bv: <br />PURPOSE of VISIT: GeoDesign representative, Joe Dapcevich, was on -site part time at the request of Craig with DDS to <br />observe the backfill for Lots 224, 225, and 238. <br />SUMMARY of OBSERVATIONS: <br />Lot 224: <br />Crews informed GeoDesign that they had over -excavated the SE corner of Lot 224 approximately 3' and backfilled with <br />approximately 3' of quarry spalls. GeoDesign observed that non -woven geotextile fabric had been laid out on top of the <br />spalls and the rest of the over -excavated lot subgrade. GeoDesign observed that crews were in the process of backfilling <br />the rest of the over -excavation with recycled concrete to bring up to planned foundation grade. <br />Lot 225: <br />GeoDesign observed that crews had placed non -woven geotextile fabric throughout the entire over -excavated lot subgrade. <br />GeoDesign observed crews begin to backfill the lot with 2-inch railroad ballast to bring up to planned foundation grade. <br />Lot 238: <br />GeoDesign observed that crews backfilling the over -excavated lot subgrade with 2-inch railroad ballast to bring up to <br />planned foundation grade. <br />The contractor was notified of GeoDesign's observations prior to GeoDesign's departure from the site. Craig informed <br />GeoDesign that crews plan to place all remaining backfill material in lifts and compact with a roller.
