City of Everett Human Needs Grant <br /> January 1 through December 31, 2018 <br /> Cocoon House <br /> Cocoon House Street Outreach and Day Center <br /> EXHIBIT A: Program Summary <br /> Cocoon House Street Outreach and U-Turn Drop-In Center serve homeless and at-risk <br /> youth ages 12 through 24 with the goal of reducing youth homelessness and assisting <br /> youth in improving self-sufficiency. The Outreach programs provide case management, <br /> mobile and stationary outreach, supported connections to community providers, and <br /> other services and referrals. U-Turn Drop-In Center operates seven days a week <br /> providing meals, showers, laundry facilities, basic needs support, life skills training, <br /> connection to community resources, and individualized case management. <br /> Measurable Outputs / Outcomes <br /> • At least 332 Everett residents engage in Outreach services <br /> • At least 6,640 units of basic needs items are distributed <br /> • At least 60% of youth engaged in case management make progress on safe <br /> housing goals <br /> • At least 75% of youth engaged in case management make progress on positive <br /> youth development goals (including education, employment, life skills, and health <br /> goals) <br />
