City of Everett Human Needs Grant <br /> January 1 through December 31, 2018 <br /> Friends of Youth <br /> New Ground Everett Transitional Living Program <br /> EXHIBIT A: Program Summary <br /> (1) Program Description <br /> New Ground Everett(NGE) provides 12 units of transitional housing and on-site supportive services <br /> to youth, ages 18-21, who are experiencing homelessness. Residents live in shared, two-bedroom <br /> apartments and may stay in the program for up to 18 months. All residents are homeless, single, <br /> between the ages of 18-21 at entry, and unprepared for independent living—with incomes less than <br /> 30% of the area median income. Prior to entering the program, residents are either living on the <br /> street, in a camp, at a shelter, or in a temporary living arrangement. During their stay, residents work <br /> with their Case Manager to create an Independent Living Plan (ILP) that defines the goals they want <br /> to work on while in the program, including permanent housing, employment, education, life skills, <br /> financial stability, health and well-being. Weekly case management meetings-help residents steadily <br /> work toward their goals, and they review their progress and set new goals with their Case Manager <br /> every three months. In addition, our Employment-Focused Case Manager provides on-site <br /> employment services, job search assistance and work readiness skills development. <br /> (2) Program Objective <br /> To provide housing and supportive services to homeless youth, ages 18-21 at entry, so that they can <br /> develop the skills they need to live independently and build self-sufficiency in their lives. <br /> (3) Program Components and Structure <br /> (a) Supportive Transitional Housing <br /> NGE is part of a condominium complex with four buildings located in the safe, residential <br /> neighborhood of Silver Lake. NGE includes 12 units of transitional housing. Residents live in <br /> shared two-bedroom apartments. The program benefits homeless youth by providing a safe <br /> home and the support needed to develop independent living skills, pursue education and <br /> employment goals, increase social skills, manage life's stresses, and successfully move into <br /> stable housing. Many of the young people we serve have never lived independently, and, like <br /> many of their peers, lack experience and skills in household planning, budgeting and time <br /> management. At New Ground Everett, youth succeed through the support of our caring staff, <br /> through building relationships with their peers, and by being part of a community. <br /> (b) Case Management <br /> When a resident first moves in, we focus on helping them adapt to their new home and a new <br /> routine while building trust with program staff, their roommates and neighbors. We use evidence- <br /> based screening tools, including the Arizona Self Sufficiency Matrix and the Ansell-Casey Life <br /> Skills Assessment, to assess the needs and strengths of each young person. The results of these <br /> assessments form the basis for discussions between the Case Manager and each client to <br /> develop an Independent Living Plan (ILP) with achievable, time-bound goals on topics including <br /> permanent housing, employment, life skills and personal finances. <br /> New Ground Everett provides a safe, supportive and temporary home where residents can start <br /> building healthy daily routines and make long-term plans. Youth are the decision makers <br /> concerning their goals and work closely with the Case Manager to adjust goals and action steps <br /> as needed through weekly meetings. Residents contribute to discussions about issues affecting <br /> the group at regular community meetings of all clients. Through service learning (volunteer) <br /> activities, our residents have opportunities to give back to others, learn that they can have a <br />
