City of Everett Human Needs Grant <br /> January 1 through December 31, 2018 <br /> Catholic Community Services <br /> CHART Case Management <br /> EXHIBIT A: Program Summary <br /> Provide housing support services to twenty (20) extremely low-income, Literally Homeless and at <br /> Imminent Risk of Homelessness households selected from the CHART List by the City of Everett. The <br /> CHART case manager will primarily focus on priority needs of the identified CHART individuals to help <br /> reduce contacts with criminal justice and emergency response systems. The primary intervention is <br /> facilitating necessary services that will support an individual to reside indoors. The expected roles and <br /> activities of the case manager will include but are not limited to: <br /> • Act as an essential member of a multi-disciplinary team providing on-going case management <br /> services with a harm-reduction focus to adults with severe and persistent mental illnesses and drug <br /> and alcohol-related issues. <br /> • Provide outreach and engagement services to assigned clients, to initially engage clients and <br /> maintain working relationships. <br /> • Conduct psychosocial assessments; develop goal planners with consumer participation; facilitate <br /> linkages to collaborative resources when appropriate. <br /> • Assume primary responsibility for coordinating all aspects of consumers' support and goal planners. <br /> This includes: providing and/or coordinating the delivery of psychiatric treatment; substance abuse <br /> treatment; and performing acute case management to facilitate consumer access to food, clothing, <br /> housing, medical care, financial assistance and vocational training or employment. Work with other <br /> services providers to provide robust support and a plan for off hour needs of the client. <br /> • Advocate for consumers' access to community resources and services, ensuring that consumers' <br /> needs are met and rights are maintained; consult and collaborate with community providers to <br /> ensure continuity of care. <br /> • Transport clients by providing or arranging transportation to necessary appointments that meet <br /> client goals. <br /> • Participate in the planning, organizing and facilitating of weekly CHART meetings presenting updates <br /> on all active clients. <br /> • Travel locally to outreach consumers; meet them on the street/in the community, make home visits, <br /> and assist with home inspections. <br /> • Provide, at times, crisis intervention and risk assessment, which could require tolerating unusual or <br /> unpleasant behaviors. <br /> • Document services provided. <br />
