City of Everett Human Needs Grant <br /> January 1 through December 31, 2018 <br /> Housing Hope <br /> Homeless Teen and Young Parent Program <br /> EXHIBIT A: Program Summary <br /> Program Objective: <br /> 1. Assist homeless teen parents in maintaining their housing or moving to other <br /> permanent housing in the community. <br /> 2. Assist homeless teen parents in increasing their earning potential, including <br /> completing high school or a GED program, advanced educational program or <br /> job training program. <br /> 3. Assist homeless teen parents in increasing their total household income, _ <br /> including earned income from employment and public benefits as appropriate <br /> and eligible. <br /> 4. Assist homeless teen parents in increasing their household management skills, <br /> including parenting skills that will ensure healthy child development. <br /> Program Components and Structure: <br /> Housing Hope offers a comprehensive program designed to assist these very young <br /> homeless families to gain the competencies, assets and confidence to transition to their <br /> adult roles and responsibilities, maintain permanent housing and increase levels of self- <br /> sufficiency. Family Support Coaches in this project focus on helping these parents <br /> complete their adolescent development, complete their basic education, establish a <br /> strong foundation of employment, acquire life skills, and parent their very young <br /> children. The primarily role of the Family Support Coach is to provide a trusting, <br /> consistent and supportive adult relationship that becomes the launching point from <br /> which these teens build connections with the community and gain the confidence to <br /> view themselves as capable adults. Family Support Coaches support these young <br /> parents to create a vision for their adult lives and to develop an Individualized Family <br /> Plan, outlining the specific long and short-term goals for permanent housing and self- <br /> sufficiency. Through in-home services and connections to community services, Family <br /> Support Coaches help families learn new skills, change attitudes, and complete <br /> activities that move them progressively towards the achievement of their long-term <br /> goals. Services are tailored (type, frequency, and amount) to meet the unique needs of <br /> each family. <br />
