EXHIBIT A <br /> SCOPE OF WORK <br /> The purpose of this contract is a hazardous materials assessment for the South Precinct-911 lease space and <br /> adjacent storage area located at 1121 SE Everett Mall Way.Each area is approximately 5,000 square feet and are <br /> located adjacent to each other at the southwest corner of the second floor. <br /> Summary of the Scope of Work <br /> The work shall incude the required pre-renovation Asbestos Good Faith Inspection,AGFI,including random lead <br /> paint sampling and a light fixture inventory so that the hazardous materials:asbestos,lead,mercury and <br /> polychlorinated biphenyls,PCB,can be identified for worker protection during any work activity that may disturb <br /> these materials. <br /> A breakdown of the Scope of Work is as follows: <br /> 1. Asbestos: <br /> a. Conduct a Asbestos Good Faith Inspection-AET RFP 3171. <br /> b. Sampling and analysis of an estimated 25 PLM Samples at$15 each. <br /> c. Sampling and analysis 5 point count 400 samples at$65 each. <br /> d. Cost Estimate-$1,500 <br /> 2. Lead: <br /> a. Ramdom lead paint samplings and analysis- 10 samples at$35 each. <br /> b. Lead results included with AGFI report. <br /> c. Cost Estimate-$750 <br /> 3. Hazardous Chemicals: Hg&PCB <br /> a. Flourescent light fixtures tubes(Hg)and Ballasts(PCB,Polychlorinated Biphenyls). <br /> b. Light fixture component inventory with AGFI report. <br /> c. Cost Estimate-$500 <br /> 4. Time for Completion: <br /> a.
