' s r 1 b. The business name, address and telephone number of the establishment. 2 c. The names, addresses, telephone numbers, and social 3 security numbers of any partners, corporate officers, share- holders who own ten percent or more of the business, or 4 other persons who have a substantial interest or manage- ment responsibilities in connection with the business, 5 specifying the interest or management responsibility of each. For the purpose of this subsection "substantial 6 interest" shall mean ownership of ten percent or more of the business, or any other kind of contribution to the business of 7 the same or greater size. 8 d. The name, address and telephone number of the owner of the property on which the public place of adult entertain- 9 ment is located. 10 2. Application for Manager or Entertainer License. 11 All applications for a manager's or entertainer's license shall be 12 signed by the applicant and notarized or certified to be true under penalty of perjury. At the time of application a picture shall be 13 taken of the applicant by the City Clerk. All applications shall be submitted on a form supplied by the City Clerk, which shall require 14 the following information: 15 a. The applicant's name, home address, home telephone number, date and place of birth, social security number, and 16 any stage names or nicknames used in entertaining. 17 b. The name and address of each business at which the applicant intends to work as an entertainer. 18 c. With the application the applicant shall present picture 19 identification which shall include (i) a motor vehicle operator's license issued by any state bearing the applicant's 20 photograph and date of birth; or (ii) an identification card bearing the applicant's photograph and date of birth. 21 d. The City Clerk shall issue the license promptly upon receipt 22 of the application, the information required to be in the application, the investigation by the Police Department in 23 accordance with Section 1(E)(4), and the license fee. 24 3. If any person or entity acquires, subsequent to the issuance of a public place of adult entertainment license for places offering 25 adult entertainment, a substantial interest, as defined in Section 1(E)(1)(c), in the licensed premises, notice of such acquisi- 26 tion shall be provided in writing to the City Clerk forthwith. The information required to be provided pursuant to this section shall 27 be that information required pursuant to Section 1(E)(1)(c) of this chapter. 28 4. The Clerk, upon presentation of such applications and before acting 29 upon the same, shall refer such applications to the Police Department, which shall make a full investigation as to the truth 30 of the statements contained therein. 31 32 -3-
