5. Long term disability insurance providing for <br /> payment of an amount equal to 2/3rds of the <br /> Appointive Officer' s annual salary not to exceed <br /> $2,000.00 monthly until age 70. <br /> 6. Optional dependent life insurance covered at the <br /> Appointive Officer' s expense . <br /> 7. Annual physical examinations for all department <br /> heads. <br /> 8. A vision care plan for the Appointive Officer and <br /> his/her eligible dependents. <br /> 9. A chiropractic care plan for the Appointive <br /> Officer and his/her eligible dependents. <br /> C . The following days are hereby designated paid holidays <br /> for those Appointive Officers in pay status on the day <br /> before and the day after the holiday : <br /> New Year ' s Day <br /> Lincoln' s Birthday <br /> Washington' s Birthday <br /> Memorial Day <br /> Independence Day <br /> Labor Day <br /> Veteran' s Day <br /> Thanksgiving Day <br /> Day After Thanksgiving Day <br /> Christmas Day <br /> One Floating Holiday <br /> D. Recognizing that an annual vacation is of benfit to both <br /> the Appointive Officers and the City, such Appointive <br /> Officers, not including Department Heads, shall accrue <br /> vacation credit for each month of continuous service as <br /> shown on the table below: <br /> VACATION CREDIT ACCRUED <br /> Continuous Service From NUMBER OF WORK DAYS <br /> Adjusted Employment Date Per Month Per Year <br /> First Year 1-1/6 days 14 <br /> Second Year 1-1/4 days 15 <br /> Third and Fogrth Years 1-1/3 days 16 <br /> Fifth Year 1-5/12 days 17 <br /> Sixth and Seventh Years 1-1/2 days 18 <br /> Eighth and Ninth Years 1-7/12 days 19 <br /> 10th through 14th Years 1-2/3 days 20 <br /> 15th through 19th Years 1-5/6 days 22 <br /> 20th Year and Beyond 2 Days 24 <br /> Department Heads shall accrue vacation at the rate of two <br /> (2) days per month or 24 days per year. <br /> No vacation credit accrual will be allowed in excess of <br /> two (2) full years accrual. <br /> EXCEPTION: Effective January 1, 1986, no employee shall <br /> have vacation credit accrual in excess of thirty (30) <br /> days at the time of his/her retirement/separation. In <br /> the event that an employee elects to retire, all vacation. <br /> accrual in excess of thirty (30) days must be taken prior <br /> to the effective retirement/separation date or be lost to <br /> the employee. <br /> No vacation shall be granted until the Appointive Officer <br /> has completed a minimum of six (6) months of City service <br /> without prior permission of the Mayor. <br />
