EXHIBIT A <br /> PROJECT DESCRIPTION <br /> COLBY AVENUE YOUTH CENTER <br /> I. PROJECT DESCRIPTION <br /> The Agency shall develop a new multi-service facility to provide housing, a drop-in center, a <br /> kitchen and dining area, and social service and administrative areas at 3530 Colby Avenue, <br /> Everett, further described in the legal description attached as Attachment A. The residential <br /> portion of the Project will provide 40 units of supportive housing for youth and young adults <br /> who are homeless or at risk of homelessness in Snohomish County. <br /> II. PROJECT CONFIGURATION <br /> The Colby Avenue Youth Center will be three stories tall with the first floor containing the <br /> outreach and prevention programs and the organization's administrative offices. The primary <br /> or shared program areas on the first floor will include the lobby, the drop-in center, the <br /> counseling and services areas, and Cocoon House's (CH's) administrative offices. The <br /> second and third floors will contain all 40 of the single room occupancy (SRO) housing and <br /> studio type units. The 31,848-square foot Youth Center will be made up of (1) housing (61% <br /> of square footage), and (2) the drop-in center, social service spaces and CH's administrative <br /> home office (aggregate 39% of square footage). Agency shall only use the HOME funds to <br /> support the housing component of the facility. By co-locating the housing with the drop-in <br /> center and other CH service programs, housing residents will have access to a wider range <br /> of amenities than if the housing were a stand-alone program. <br /> III. RESIDENT POPULATION <br /> a. At project completion, all forty (40) units will be affordable to households at or <br /> below 30% of Area Median Income (AMI). <br /> b. Agency shall ensure that twenty single room occupancy units of transitional <br /> housing, each with their own bathroom, will serve homeless youth, aged 15-17. <br /> This age group will be housed on a separate floor with a secure entrance in three <br /> separate dormitory-style wings, each wing with its own community living room and <br /> kitchen area. This is transitional housing with residency allowed for 21 months or <br /> until an individual's 18th birthday as required by licensing and funding <br /> requirements. <br /> c. Agency shall ensure that twenty studio units of permanent supportive housing, <br /> each with their own bathroom and kitchenette, will serve young adults, aged 18-24 <br /> who are homeless (literally or at-risk of homelessness.. These twenty units will be <br /> separated into two dormitory style wings, each wing with its own community living <br /> room and kitchen area. Although the housing will be permanent, the Agency shall <br /> ensure that residents are prepared to move to conventional housing as soon as <br /> they are ready to transition to self-sufficiency. <br /> Exhibit A <br /> Cocoon House <br /> Page 1 of 7 <br />
