No vacation shall be granted until the Appointive Officer <br /> has completed a minimum of six (6) months of City service <br /> without prior permission of the Mayor. <br /> E. The City shall grant Appointive Officers leave of absence <br /> with pay for the following reasons: <br /> 1. Funeral Leave : When approved by the Mayor, <br /> Appointive Officers shall be granted reasonably <br /> necessary time off with pay to assist with funeral <br /> arrangements and to attend funeral services. <br /> 2 . Sick Leave Upon employment with the City , <br /> Appointive Officers shall have available immediately <br /> ten (10) days of sick leave with pay . All <br /> Appointive Officers shall accrue sick leave at a <br /> rate of two (2) days per month to a total maximum of <br /> one hundred and twenty ( 120) work days. While still <br /> actively employed with the City of Everett , should <br /> any monthly disability payment be made to the <br /> Appointive Officer as a benefit resulting from any <br /> insurance coverage provided by the City , the City <br /> shall only pay the difference between the amount of <br /> the benefit and the Appointive Officer ' s normal <br /> monthly salary . In no case, shall such sick leave <br /> payment exceed one hundred and twenty (120) work <br /> days at eight (8) hours per day . <br /> Section 2 : City Council : <br /> A . Members of the City Council of the City of Everett who <br /> request coverage under the City ' s medical insurance plan <br /> shall be granted such coverage to be comprised of the <br /> following : <br /> 1. Basic and major medical coverage for the <br /> Councilmember and his/her eligible dependents. <br /> 2 . A dental plan for the Councilmember and his/her <br /> eligible dependents. <br /> 3 . A vision care plan for the Councilmember and his/her <br /> eligible dependents. <br /> 4. The face amount of life insurance and principle sum <br /> of accidental death and dismemberment insurance for <br /> a Councilmember under age 70 shall be at an amount <br /> equal to two times the Councilmember ' s basic annual <br /> earnings , rounded to the next higher mulitple of <br /> $1 ,000 plus $2 ,000. The maximum face amount and <br /> maximum principle sum shall each be $150, 000. <br /> 5. Long term disability insurance providing for payment <br /> of an amount equal to 2/3rds of the Councilmember ' s <br /> annual salary not to exceed $2 , 000.00 monthly until <br /> age 70 . <br /> 6 . The face amount of life insurance and the principle <br /> sum of accident insurance for a Councilmember age 70 <br /> or over shall each be equal to one-half the <br /> applicable amounts determined above. <br /> 7. Optional dependent life insurance covered at the <br /> Councilmember' s expense . <br />
