<br /> The various permits obtatned for the project are on file m the offlces of the engineer.
<br /> 2 Miscellaneous structures and obstructions such as sign posts,matl boxes,meter boxes,or other obstructions,shall be
<br /> awided or removed and reinstalled to original or better condtion.
<br /> 3.The contractor shall be responsible,at all times; for the maintenance of the streets or other utllitles affected by
<br /> canstruction operations—debris shall not be permftted to accumulate and all premises shall be maintained in a neot workmon
<br /> Ike conditian.
<br /> 4.The contractor shall take adequate precautions to pratect exfsting faclities,sidewalks,curbs,pavements,uUfities,fdiage,
<br /> adjolning property and structures autside of right—of—way,and to avoid damage there to.
<br /> 5.The following special provisfons sholl apply to traffic regulation during the extent of this project
<br /> o.There shall be,at all tlmes,adequote vehicle and pedestrian access for ingreu and ecgess for the propertfes adJacent to

<br /> b.During non—waking hours,the contractor shall keep the existing traffic lanes dear Gom interference induding all
<br /> approaches and intersecttons.
<br /> c.If lane blodcage is unavoidable,the contractor shall notify the traffic engineering departments,police deportments,flre
<br /> departments,ambulance services,schod bus garages and other agency operations as appropriate so these agencies may
<br /> re—route their vehides around the construction zone(s).
<br /> 6.Where hazardous conditions exist; proper signmg,flagging and barricading shall be provided as directed by the engineer.
<br /> Supplemental signs,flag persons and barricades shall be provided by the contractor.
<br /> 7.Trenches in paved areas shall be saw—cut to the full depih if surfaang or as required by the permitting authority.Paved
<br /> surfaces shall be repaired in axadance with the permitting authoritys speaficatiana
<br /> &The contructor shall comply with all arrent and appl"�cable Occupational Safety and Health Administration(OSHA)federal,
<br /> state and local rules aid regulations goveming the safety of wnployees and material during excavation,trenching,ditching,
<br /> and installation and backfilling operations on this project
<br /> 9.As roqufred,a waste site shall be designoted for the disposal of excess mataials and debris.
<br /> 10.No trench or excavatian site shdl be left open overnight or unattended.
<br /> 71.Right—of—way restoration and street and sidewalk repair/replacement shall be in compliance with the local
<br /> permittngautharity.

<br /> 13.When it is warranted to use dielectrfc fiber cable is an underground situation with no metalltc canduit,a solid copper
<br /> tracer wire of 12 AYlG gauges must be pulled and placed along wfth the dielectric

<br /> cable in the sane conduil This copper tracer wire must be grounded at each acce�point(manhde,handhde or ped)and
<br /> shull serve 1he same purpose as the metal contnuity of armored cables to allow for dectronic location during plant
<br /> protection.
<br /> 74.Innerducts ocapied by the cable shall be sealed at termination points usmg rubber plugs and compression band� vacant
<br /> innerducts or canduit runs shall be sealed at termination points wlth a rubber plug equipped with a pull Iine attachment ring
<br /> on whtch to terminate the pull line.Approprfate seal�t compound shall be used to seal any gaps.
<br /> 75.Innerducts must be smooth bore pdyethylene with a mhimum I.D.of 1.25'.
<br /> 16.Fber optic cable and/or duct placed by plowing or trenching methods shall hare marker tape placed twdve inches(12')
<br /> drectly above.
