::.!;::„,,,,,,,,,,„.7.7
Parks & Community Services
MEMORANDUM
Date: May 1, 2018
To: Sharon Fuller, City Clerk
From: Lori Cummings, Director
Re: USDA Application for License - Renewal
The Forest Park Animal Farm has been licensed by the United States Department of
Agriculture (USDA), Animal Plant Health Inspection Services (APHIS) since 2005.
Attached is the 2018 license application to route for signatures. The Mayor, City Clerk
and City Attorney have signed this application in the past. I have attached an example
of the signed 2017 application for your reference.
Our license is up for renewal for the 2018 season on May 26, 2018. Please return the
signed application to me for processing. I hope that I can mail the application no later
than May 21, 2018 to ensure the USDA receives it by our renewal date along with
payment. If you return the application to me I will ensure that occurs.
The APHIS Animal Care program enforces the Animal Welfare Act (AWA) through
inspections of regulated facilities. Qualified animal care inspectors conduct
unannounced inspections of licensed facilities. This has proven to be advantageous to
the City.
If you have any additional questions, please contact Scott Pool at ext. 7307 or Jan
Tanner at ext. 8347. Thank you for your assistance in processing this Licensing Renewal
Application.
