PUBLIC NfOR/C5 sente.mati <br /> tz-//40 <br /> February 11, 2016 (Updated) <br /> HOUSING AUTHORITY OF EVERETT <br /> 3107 COLBY AVE <br /> EVERETT, WA 98201 <br /> Complaint: LACKS ACCESSIBLE PARKING <br /> Job Address: 1115 RAINIER AVE <br /> Complaint No: W1601-006 <br /> RE: PARKING LOT STRIPING, WORK WITHOUT PERMITS <br /> Dear HOUSING AUTHORITY OF EVERETT, <br /> A complaint was received on January 11th, 2016, about the lack of accessible parking to service <br /> the building(s) at the address listed above. <br /> Permit History <br /> Attached site plan indicates that parking lot alterations have been made without permits. <br /> Parking Lot Revisions-Permit Application Required <br /> A building permit application is required to indicate how the accessible parking will be restored, <br /> including signage. To discuss this letter, please contact me at mhaley©everettwa.gov or 425- <br /> 257-8813. A new permit application and two sets of drawings should be submitted in a timely <br /> manner such that the permit can be issued within 90 days of the date of this letter, no later <br /> than May 11th, 2016. Final inspections must occur within 180 days of the issuance of the <br /> permit. <br /> All required submittals must be of sufficient detail and information to ensure that the proposed <br /> structure will be restored to its original permit configurations, or updated to be in compliance <br /> with current adopted codes. Please note that federal ADA, FHA, and Fair Housing Accessibility <br /> Guidelines and regulations may be more restrictive. Compliance with these regulations is the <br /> responsibility of the owner of the building. <br /> These comments address building issues only.
